神经外科机械通气性肺炎病原学分析

摘要 目的 了解神经外科机械通气性肺炎 (ventilatorassociatedpneumoniaVAP)病原菌感染的现状与分布 ,为临床减少、控制VAP的发生、传播 ,合理使用抗生素提供依据。 方法 对 2 0 0 2年我院神经外科采用机械通气患者肺部感染进行回顾性调查 ,对致病菌的分布及易感因素进行分析。 结果  10 1例机械通气患者中VAP患者 76例 ,VAP发生率为 75 .2 % ,主要致病菌分布为绿脓杆菌 (3 4.3 % )、金黄色葡萄球菌 (13 .8% )、凝固酶阴性的葡萄球菌 (10 .5 % )、不动杆菌(10 .5 % )等。 结论 神经外科机械通气患者VAP发生率高 ,医务人员应强化消毒隔离、无菌操作技术 ,合理使用抗生素。 Objective To understand the situation and distribution of bacterium in ventilator associated pneumonia (VAP) in neurosurgery, provid a reasonable selection for antibiotic use in clinic to decrease and control the occurrence and emission of VAP. Methods Retrospective studies were made to analyze the distribution and predisposing factors of pathogenic bacterias on lung-infected patients using mechanical ventilation in neurosurgery in 2002 in our hospital. Result In 101 patients investigated having used mechanical ventilation, 76(75.2%) were infected with VAP,the major bacteria were bacillus Pseudomonas aeruginosa(34.3%),Staphylococcus aureus(13.8%),SCN(staphylococcus coagulase test is negative)(10.5%),and Acinetobacter(10.5%). Conclusion High VAP incidence happened in neurosurgery patients using mechanical ventilation.It is important to strengthen aseptic techique and select reasonable antibiotics.
