摘要
目的比较血管内栓塞和显微手术治疗低级别大脑中动脉动脉瘤(middle cerebral artery aneurysm,MCAA)的临床疗效。方法将66例低级别MCAA患者分为显微手术治疗组(42例)和血管内栓塞治疗组(24例),对两组疗效、并发症、瘤颈残余率、住院时间、手术时间及住院费用等进行分析比较。结果本研究中显微手术治疗组42例,其中25例(59.52%)恢复良好,1例(2.38%)死亡;血管内栓塞治疗组24例,其中23例(95.83%)恢复良好,无死亡病例。血管内栓塞组手术并发症发生率(16.7%)低于显微手术组(40.5%)(P<0.05)。血管内栓塞组住院时间和手术时间均低于显微手术组,住院费用高于显微手术组(P<0.05)。结论血管内栓塞治疗低级别MCAA的手术操作时间和住院时间短、恢复快、预后好、术后并发症少,若动脉瘤形态适宜且病人经济条件许可,可作为治疗低级别MCAA的首选方式。
Objective To compare the clinical curative effect between the endovascular treatment and microsurgery in low grade middle cerebral artery aneurysm. Methods The data on the treatment of 66 patients of low grade middle cerebral artery aneurysm were reviewed. All patients were assigned to microsurgical group( n = 42) and endovascular group( n = 24). Aneurysm residual rate,complications,hospitalization time,operation time,and the hospitalization cost were analyzed and compared between two groups. Results Of 42 cases in microsurgical group,25 cases( 59. 52%) recovered well after operation and 1 case( 2. 38%) died.Of 24 cases in endovascular group,23 cases( 95. 83%) recovered well after operation and 0 case died. Surgical complication rate( 16. 7%) of endovascular embolization group was significantly lower than that of the microsuogery group( 40. 5%)( P < 0. 05). The hospitalization time and operation time of endovascular embolization group were lower than those of microsurgical group. Conclusion Endovascular embolization has shorter operation time and hospitalization time,more rapid recovery,better prognosis and less postoperative complication than microsurgical group has. So endovascular embolization can be used as the preferred treatment for patients with the low grade middle cerebral artery aneurysm with well morphology of aneurysm for embolizing and good economic conditions.
