摘要
目的 分析肌袖性房颤中房性心律失常的动态心电图特点。方法 选择肌袖性房颤患者 4 0例 ,非房颤患者 4 6例。分析 2 4小时动态心电图。结果 肌袖性房颤组见 :频发短阵房颤、房扑。房颤发作平均持续时间 2 5 6± 7 5秒 ,房颤发作前与房颤相关的房早联律间期 0 4 5± 0 0 7秒。对照组 :4 6例动态心电图记录中无房颤、房扑发生 ,房早联律间期 0 6 5± 0 14秒。肌袖性房颤组房颤发作前与房颤相关的房早及未下传的房早的联律间期明显短于对照组房早联律间期 (P <0 0 1)。结论 肌袖性房颤中房性心律失常有着一定规律和心电图特点 ,DCG中有着频繁的窦律、房扑、房颤相互转换 ,均伴频发的房早 ,房早几乎均呈“PonT”现象 ,且房早易伴室内差异传导及房早未下传、P′-R延长 ,房颤大多由“PonT”房早触发或驱动 ,部分房颤由房扑触发 ,常可有短阵 3～ 6个连续扑动波。通过动态心电图监测可确定导管射频消融合适病例 ,并随访评价消融效果。
Objective To analyze the characters of Holter monitoring electrocardiogram recorded in atrial arrhythmia relative to muscle sleeve atrial fibrillation.Methods 24 hour Holter monitoring electrocardiograms of 40 patients with atrial fibrillation(AF) and 46 patients without atrial fibrillation were analyzed.Results The mean duration of AF was 25.6±7.5 seconds.The interval of atrial premature beats that were not relative to AF was 0.45±0.07 second.There was no AF or (AFL) detected by Holter monitoring electrocardiogram in the 46-cases group for comparison.The interval of atrial premature beat was 0.65±0.14 second.The intervals of atrial premature beats before muscle sleeve AF and those failing to transfer were obviously shorter than those of comparison group ( P <0.01).Conclusions There are some laws and characters of electrocardiogram in atrial arrhythmia relative to muscle sleeve atrial fibrillation.Frequent transition of sinus beats,AFL and AFcould be seen,all accompanied with frequent atrial premature beats,which are 'PonT' frequently.Furthermore,the premature beats often induce discrepant conduction or fail to conduct.P'-R intervals are longer.AF is always triggered by 'PonT' atrial premature beats.Some may be by AFL.Holter monitoring electrocardiogram can serve as a means of selecting patients for radio-frequency ablation as well as for following-up examination.
