摘要
目的:探讨改曼术及阴道半封闭术治疗老年性重度盆腔脏器脱垂(POP)的临床疗效。方法:收治重度POP患者22例,分别行改曼术(阴道前后壁修补术+宫颈截除术+主韧带短缩术)及阴道半封闭术治疗,回顾性分析其临床资料。结果:两组患者均安全度过围手术期,顺利恢复自主排尿,阴道半封闭组出现1例阴道血肿,改曼术后2例复发,后行阴道半封闭术。结论:阴道半封闭术能缓解多数患者已有的排尿、排便症状,对于无阴道性生活要求的老年重度POP患者是一种安全、经济、疗效满意的术式,但对于有明确性生活要求的老年患者,改曼术是较好的选择,但其较易复发。
Objective:To explore the clinical effect of elderly patients with severe pelvic organ prolapse(POP) with the treatment of modified mann type operation and vaginal semi closed surgery.Methods:22 patients with severe POP were selected.They were given modified mann type operation(the anterior and posterior vaginal wall repair plus cervical resection plus main ligament shortening) and vaginal semi closed surgery respectively,and the clinical data were retrospectively analyzed.Results:Two groups of patients were safely through the perioperative period.The autonomous urination was recovery smoothly.In vaginal semi closed surgery group,1 case had vaginal hematoma.In modified mann type operation group,2 cases had recurrence,and then,they were given vaginal semi closed surgery.Conclusion:Vaginal semi closed surgery can relieve the symptoms of urination and defecation in most patients.For the elderly patients with severe POP without the requirement of vaginal sex life,it was a safe,economical and satisfactory operation,but for the elderly patients with severe POP with the requirement of vaginal sex life,modified mann type operation was a better choice,but it was more prone to relapse.
