摘要
目的:对比分析腹部X线、超声、螺旋CT诊断肠梗阻的临床效果,以确立肠梗阻早期诊断的确切方案。方法:2012年8月-2013年8月收治肠梗阻患者265例,比较X线腹部检查、腹部超声、螺旋CT对于肠梗阻、梗阻部位、梗阻原因及是否发生肠绞窄的诊断符合率。结果:X线、超声、CT检查对诊断肠梗阻、梗阻部位、梗阻原因以及肠绞窄差异有统计学意义(P<0.05);超声、CT检查对于肠梗阻诊断符合率分别可达93.2%、97.4%,较腹部X线检查(70.6%)显著升高,对于梗阻部位、梗阻原因以及肠绞窄的诊断符合率也显著提高(P<0.01);超声及CT检查对比发现,CT检查对于肠梗阻的各项诊断符合率均明显升高(P<0.05)。结论:超声、CT检查对于肠梗阻的诊断符合率较腹部X线检查显著提高,以CT检查结果最优。鉴于肠梗阻需实时动态监测,超声诊断更适合肠梗阻诊断的临床应用。
Objective:To compare and analyze the clinical effect of abdomen X-ray,ultrasound,spiral CT in the diagnosis of intestinal obstruction,to establish the exact solution of the early diagnosis of intestinal obstruction.Methods:265 patients with intestinal obstruction were selected from August 2012 to August 2013.The accordance rates of X-ray abdominal examination,abdominal ultrasound and spiral CT in the diagnosis of intestinal obstruction,obstruction position,obstruction reason and whether intestinal strangulation occur.Results:X-ray,ultrasound,spiral CT in the diagnosis of intestinal obstruction,obstruction position,obstruction reason and intestinal strangulation had significant difference(P<0.05).The accordance rates of ultrasound and spiral CT in the diagnosis of intestinal obstruction were respectively 93.2% and 97.4%,there were significantly increased than 70.6% of X-ray abdominal examination.The accordance rates in the diagnosis of obstruction position,obstruction reason and intestinal strangulation were also significantly improved(P<0.01).Ultrasound and CT were compared found that the accordance rates of CT examination in the diagnosis of intestinal obstruction were obviously increased(P<0.05).Conclusion:The accordance rates of Ultrasound and CT examination in the diagnosis of obstruction position were significantly improved than that of X-ray abdominal examination,the CT examination result is the best.In view of intestinal obstruction needs real-time dynamic monitoring,the ultrasonic diagnosis is more suitable for clinical application in the diagnosis of intestinal obstruction.
