摘要
目的:探讨尿激酶静脉溶栓治疗脑梗死的临床疗效。方法:2013年2月-2014年6月收治脑梗死患者80例,随机分成对照组和观察组,各40例,对照组给予拜阿司匹林100 mg/d口服,法安明5 000 U皮下注射,2次/d,连续治疗2周,观察组在对照组的基础上加尿激酶75万U+100 m L 0.9%氯化钠注射液静滴,30 min内滴完,1 h后同样剂量的尿激酶同样时间滴完,连续治疗2周。结果:两组患者治疗前ESS评分比较差异无统计学意义(P>0.5),治疗1d、7 d和14 d,观察组明显优于对照组,两组比较差异有统计学意义(P<0.05)。治疗结束后观察组Barthel指数0~60分1例,61~70分5例,71~80分10例,81~90分12例,91~100分12例;对照组0~60分12例,61~70分12例,71~80分5例,81~90分6例,91~100分5例,两组比较差异有统计学意义(P<0.05)。结论:尿激酶静脉溶栓治疗脑梗死ESS评分和Barthel指数明显升高,疗效显著,值得推广。
Objective:To investigate the clinical efficacy of intravenous thrombolysis with urokinase in the treatment of cerebral infarction.Methods: 80 patients with cerebral infarction were selected from February 2013 to June 2014.They were randomly divided into the control group and the observation group with 40 cases in each.The control group were given aspirin 100 mg/d oral,subcutaneous injection of Fragmin 5 000 U,2 times/d,the treatment continuous for 2 weeks.The observation group were given urokinase 750 000 U+100 m L NS intravenous drip on the basis of the control group,infusion in 30 min,the same dose of urokinase drop out at the same time after 1 h,the treatment continuous for 2 weeks.Results:The ESS score of the two groups of patients before the treatment showed no statistical significance(P>0.5).After the treatment of 1 d,7 d and 14 d,the observation group was significantly better than the control group,and the two groups had statistical significance(P<0.05).The Barthel index of the observation group after the treatment was:1 case of 0~60,61~70 in 5 cases,10 cases of 71~80,81~90 in 12 cases,91~100 in12 cases;the control group was:12 cases of 0~60,61~70 in 12 cases,71~80 in 5 cases in 6 cases,81~90,91~100 in 5 cases;the two groups had statistical significance(P<0.05).Conclusion:The ESS score and the Barthel index of intravenous thrombolysis with urokinase in treatment of cerebral infarction is significantly increased,and the effect is significant,so it is worthy of promotion.
