摘要
目的:探讨CT与MRCP诊断胆石症的应用价值。方法:2011年12月-2013年4月收治胆石症患者96例,与术前检查诊断做对照分析,术前检查CT及MRCP。结果:诊断肝内胆管结石,MRCP的灵敏度92%,CT的灵敏度78%,差异具有统计学意义(χ2=4.16,P<0.05);诊断胆囊结石MRCP的灵敏度94%,CT的灵敏度87%,差异无统计学意义(χ2=0.21,P>0.05);诊断胆总管结石MRCP灵敏度95%,CT的灵敏度76%,差异具有统计学意义(χ2=4.03,P<0.05)。诊断胆石症MRCP的灵敏度94%,CT的灵敏度78%,χ2=7.57,P<0.01。MRCP曲线下面积0.954 48,标准正态离差z=0.09,置信区间(0.935 48,0.974 8);CT曲线下面积0.874 21,标准正态离差z=0.045,置信区间(0.784 21,0.964 21)。结论:通过对比CT、MRCP的诊断效果,MRCP在诊断胆石症方面明显优于CT,值得临床推广应用。
Objective:To explore the application value of CT and MRCP in the diagnosis of cholelithiasis.Methods:96 cases with cholelithiasis were selected from December 2011 to April 2013.They were compared with preoperative examination diagnosis.The preoperative examination was CT and MRCP.Results:In the diagnosis of hepatolith,the sensitivity of MRCP was 92%;the sensitivity of CT was 78%;there was a significant difference(χ2=4.16,P<0.05).In the diagnosis of gallbladder stone,the sensitivity of MRCP was 94%;the sensitivity of CT was 87%;there was no significant difference( χ2=0.21,P<0.05).In the diagnosis of choledocholithiasis,the sensitivity of MRCP was 95%;the sensitivity of CT was 76%;there was a significant difference(χ2=4.03,P<0.05).In the diagnosis of cholelithiasis,the sensitivity of MRCP was 94%;the sensitivity of CT was 78%,χ2=7.57,P<0.01.The area under the MRCP curve was 0.954 48;the standard normal deviate z=0.09;the confidence interval was(0.935 48,0.974 8).The area under the CT curve was 0.874 21;the standard normal deviate z=0.045;the confidence interval was(0.784 21,0.964 21).Conclusion:By comparison of the diagnosis effects of CT and MRCP,MRCP in the diagnosis of cholelithiasis is significantly better than that of CT.It is worthy of clinical application.
