摘要
目的 探讨胸腔内注入足叶乙甙 (VP - 1 6 )、顺铂 (DDP)、滑石粉混悬液治疗恶性胸腔积液的疗效。方法 病理确诊的恶性胸腔积液 6 4例 (经胸穿于胸腔内留置导管引流尽胸水后 ) ,随机分为 2组 :治疗组 3 2例 ,在胸腔内注入VP - 1 6 ( 5 0mg/m2 )、DDP( 4 0mg/m2 )、滑石粉混悬液 3～ 5g ;对照组 3 2例 ,在胸腔内注入DDP( 4 0mg/m2 ) ,均隔日重复 1次 ,共 2次。观察疗效、生活质量、生存率及毒副反应。结果 治疗组总有效率 84 %,病变进展率 3 %,与对照组 5 0 %、2 2 %相比有显著性差异 (P <0 0 5 )。Karnofsky评分大于 70分 ,治疗组较对照组显著提高 (P <0 0 5 )。治疗组 0 5a、1a、1 5a的生存率为 97%、5 9%、4 4 %,分别高于对照组的 6 3 %、5 0 %、1 9%,0 5a、1 5a两组的生存率有显著性差异 (P <0 0 5 )。治疗组的副反应中发热、胸痛高于对照组。结论 胸腔内联合注入VP - 1 6、DDP、滑石粉混悬液治疗恶性胸液是一种效果好、副反应轻、患者可耐受的方法。
Objective To evaluate the efficacy of treating malignant pleural effusion with the mixture of VP-16,DDP and tale by injection through thoracic tube intubation.Methods Sixty_four cases of malignant pleural effusion,after drainage,were randomly divided into two groups treatment group (T,32 cases),injecting VP_16(50 mg/m 2),DDP(40 mg/m 2) and tale (3～5 g) into pleural cavity;control group (C,32 cases),only DDP (40 mg/m 2),which was repeated after one day,then evaluated the efficacy,survival rate,toxicities and the quality of life.Results The respondent was 84% in T group and 50% in C group.There was statistical differences in the two groups (P<0.05).The Karnofsky score is higher in T group than in C group.The half-year,one-year,one and half-year survival rates were 97%,59%,44% in T group and 63%,50%,19% in C group.Fever and pleura_aohe are higher in T group than in C group.Conclusions The treatment using VP-16?DDP and tale through thoracic tube is effective,the side_effect is slight and is a bearable method for malignant pleural effusion.
