摘要
目的 观察小剂量卡介苗 (BCG)、丝裂霉素C(MMC)交替灌注预防浅表性膀胱癌术后复发的疗效和安全性。方法 回顾性分析自 2 0 0 0年 5月～ 2 0 0 1年 6月的 4 5例浅表性膀胱癌患者的临床资料 ,2 9例行膀胱部分切除术 ,1 6例行经尿道膀胱肿瘤电切术 (TURBt) ,术后定期应用卡介苗 6 0mg、丝列霉素C 1 0mg交替膀胱灌注 ,每周 1次共 6次 ,间歇 3个月 ,再每月 1次 ,持续 1 2个月。其中Ta、T1 期 2 2例、T2 期 2 3例。结果 所有病人随访 1 2～ 2 4个月 ,4 5例患者中 ,共有 2例复发 ,均为TURBt后T2 病人 ,总有效率 95 2 %,无全身不良反应 ,仅 2例应用BCG后出现轻度膀胱刺激症状。结论 小剂量MMC、BCG交替膀胱内灌注 ,预防浅表性膀胱癌术后复发 ,效果好、病人耐受性强、副作用小。
Objective To evaluate the alternate intravesical instillation with half dose BCG and MMC for the treatment of superficial bladder cancer.Methods The instillation was carried out among 45 cases of superficial bladder cancer.Sixteen cases were given TURBt,partial cystectomy was carried out in 29 case(The dose of MMC was 10 mg dissolved in 20 ml of N-S,the dose of BCG was 60 mg dissolved in 20 ml of N-S) and alternate travesical instillation of MMC and BCG was given once a week for 6 weeks and then stopped until after 3 months was given once a month for 12 months.Results Forty-three of the 45 patients had been free from the tumor (95.2%).Tumor recurrence occurred only in 2 patients after TURBt,transitory bladder irritation occurred in 2 cases.There was no systemic side effect observed.Conclusions Intravesical instillation BCG and MMC are effective against the recurrence of superfical bladder cancer and there is least side effect.
