摘要
目的 :探讨肺癌切除术后患者早期胸腔内DDP化疗后毒副作用情况。方法 :观察肺癌切除术后早期胸腔内DDP(80mg m2 )化疗组 (A组 ,n =2 9)用化疗药后对术后并发症的影响 ,并与对照组 (B组 ,n =32 )比较 ;观察A组的化疗副作用 ;及胸腔内早期化疗对后期常规静脉化疗的影响。结果 :与B组相比 ,A组胸腔内DDP化疗对术后并发症无差异。早期胸腔内化疗的副作用主要有胃肠道反应、WBC降低和心律失常 ,胃肠道反应、WBC降低与静脉化疗相比要小 (P <0 0 5 ,P<0 0 0 1)。心律失常则无统计学意义 (P =0 31)。A组首次静脉CAP化疗与B组的化疗毒副反应无差异。结论 :肺癌切除术后早期胸腔内化疗的不良反应轻微 ,术后并发症无差异 ,不影响后期常规静脉化疗的按时进行。早期胸腔内化疗是安全的。
Objective:To study the adverse effects of early intrapleural administr ation with cisplatin after resected lung cancer.Methods:Sixty one patients with resected lung cancer were divided into Group A and Group B.Group A(n =29)was administrated with cisplatin(80mg/m 2)2 to 3 days afte r operation.The complications and adverse effects of Group A with cisplatin were observed.Results:Compared with the Group B,there was no difference of the posto perative complication of Group A that was administrated with cisplatin.The adv erse effects with early intrapleural administratio n with cisplatin were very mild.The major effects were gastrointestinal reaction s ,leukocytopenia and arrhythmia.There was no statistic significance of arrhythmia between the effects of the early intrapleural administration and conventional c hemotherapy(P=0 31).The effects of gastrointestinal reactions and leukocytopenia were milder than those who were undergone conventi onal chemotherapy(P<0 05,P<0 001).There was no differen ce in the adverse effects of conventional chemotherapy between two groups.Conclusi on:The adverse reactions of early intrapleural chemotherapy after operation are very mild.And it doesnt enhance the postoperative complications and the advers e effects of conventional chemotherapy.Therefore,the postoperative early int rapleural chemotherapy is safe.
