摘要
目的 评价非拔牙治疗青春中晚期以上颌发育不足为主的骨性 类错颌的骨骼变化 ,探讨非拔牙治疗对恒牙列早期中度 类骨性错颌的矫正机理。方法 对 1 2例以上颌发育不足为主的青春中晚期骨性 类错颌女性患者 ,平均年龄 1 2 .5岁 ,平均 ANB角为 - 2 .5°,伴有不同程度的上颌牙列拥挤 3~ 1 1 mm。经非拔牙矫正治疗后(平均疗程 2 9个月 ) ,比较治疗前后的 X线颅面部骨骼的变化。结果 发现治疗明显促进上颌骨垂直生长 ,尤其是上颌骨后段 ,其基骨水平往前轻度发育伴腭平面的反时针旋转 ,改善了面中份高度发育不足的畸形。下颌骨体及下颌升支也有明显生长同时伴有下颌平面的顺时针旋转 ,但前后面高比维持不变 ,开张型侧面形没有进一步加重。下颌的旋转及上颌的垂直生长改善了面中部的凹陷畸形 ,使面中部变得更加丰满。前牙反的矫正是上切牙唇顷、下切牙轻度舌顷的代偿性结果。结论 非拔牙治疗能明显改善青春中晚期的上颌发育不足骨性畸形 ,并阻止
Objective To evaluate midface changes in class Ⅲ maxillary deficiencies in later pubertal growth spurts following a non extraction treatment approach.Methods A retrospective and cephalometric study was made on a group of 12 girls successfully treated,in Skeletal Ⅲ,and who had undergone non extraction therapy and cephalometric analysis for the midface changes after treatment.Results Compared with less forward growth,significant downward growth of maxillary and noticeable.Forward growth of mandibular was restricted,mandible exhibited,and downward and backward rotation was accompanied by the clock wise rotation of occlusal plane.Facial contour was maintained and overjet correction was achieved by a combination of anterior displacement of the upper incisors and posterior displacement of the lower incisors.Conclusion Non extraction technique can effectively improve the skeletal Ⅲ maxillary deficiencies in later pubertal growth spurts.
