摘要
为探讨膝关节相关力线改变与膝关节退行性骨关节病 (OA)间的相关性。采用东芝KXO 5 0 0R型X线照相机 ,对因症就诊者摄取膝关节正侧位X线片 ,得有效病例 12 3例 ,共 192膝。另设对照组 31膝 ,分别测量其胫股关节间隙、下股骨角与上胫骨角 ,所测数据经SPSS 11.0统计处理分析。结果显示胫股关节间隙均较对照组窄 ,二者有差异性 (P <0 .0 5 ) ,下股骨角及上胫骨角均较对照组增大 ,二者存在显著差异 (P <0 .0 1)或具有差异性 (P <0 .0 5 )。表明膝关节下肢力线改变与胫股关节退行性变间存在有一定的相关性 ;而通过适度的体育锻炼以增强肌肉的张力、减轻体重以及积极防治骨质疏松等 ,对于防止下肢力线的改变 ,延缓退行性骨关节病的发生发展将会起到积极作用。提示 ,连续测量OA患者下肢力线以观察其变化规律 ,进而通过采用校正其下肢力线的方法来减缓退行性变的进程 ,可能也不失为是防治胫股关节退行性变的一个途径。
The objective of the paper is to explore the relationship between the knee force line change and the knee degenerative osteoarthritis (OA). X-ray grams were taken on 123 patients (192 knees) with OA symptoms and on 31 normal knees and used as the observed and control groups, respectively; the tibiofemoral-joint spaces and the lower femoral and upper tibial angles of the two groups were measured, and the measured data were statistically treated and analyzed with SPSS II. The results showed that compared with those of the control group, the tibiofemoral-joint spaces of the observed group were narrowed with a difference (P < 0.05) and the lower femoral and upper tibial angles were enlarged with a difference or a significant difference (P<0.05 or P<0.01). That suggested showed that there is a certain relationship between the force line change of the lower extremity and the degeneration of the tibiofemoral joint; the suitable physical exercises for increasing the muscular tension and decreasing the body weight and the active prevention and treatment of osteoporosis will play an active role in the prevention of the lower extremity force line change and the delay of the degenerative osteoarthritis attack. The successive measurement of the lower extremity force line of OA patient for observing its change law and taking the corresponding measures to correct the abnormal force line and to delay OA progress may be a good way to prevent and treat the tibiofemoral-joint degeneration.
