摘要
目的 探讨压力调节容量控制(PRVC)通气治疗新生儿呼吸衰竭的疗效。方法 21例呼吸衰竭新生儿中,有Ⅰ型呼吸衰竭5例,Ⅱ型呼吸衰竭16例。予PRVC通气,初调参数:VT 6~8ml/kg,PEEP 2~6 cmH2O,FiO2 0.6~0.7,RR 50次/min.Ti0.4~0.7 s,压力水平上限25~40 cmH2O。结果平均上机时间36.3(10-120)h。治愈16例,死亡2例,3例放弃治疗,1例改用PCV,治愈率76%。并发气压伤1例,肺炎4例。结论PRVC通气治疗新生儿呼吸衰竭有较好的疗效。可减少气压伤,缩短上机时间,减少镇静剂的应用,但由于我们观察病例数较少,有待进一步研究。
Objective To explore the effect of PRVC ventilation mode on neonatal respiratory failure- Methods 21 neonates with respiratory failure including 5 type I and 16 type II were included in this study. Ventilator settings: VT6~8 ml/kg, PEEP 2 - 6 cmH2O, FiO2 0.6-0.7, Ti 0.4-0.7 sec, RR 50b/min, upper pressure limit 25 ~40 cmH2O. Results Average time of PRVC ventilation was 36.3(10-120) hours.There were 16 cases cured, 2 cases died, 3 cases abandoned, 1 case ultered with PCV in this group.The cure rates was 76 per cent for 21 patients. And there were complications, baro trauma 1 case, pneumonia 4 cases. Conclusion PRVG ventilation is effective for neonatal respiratory failure. It can decrease barotrauma, shorten time of mechanical ventilation. And the sedatives used in PRVC ventilation were also obviously reduced. The cases is not enough, more details should be further studied.
