摘要
目的：观察前方牵引矫治Ⅲ类错■对ＴＭＪ影响。方法：对１９例安氏Ⅲ类错■伴ＴＭＤ的替牙期患者，用前方牵引矫治器进行矫治。分别于治疗前、后摄双侧薛氏位闭口Ｘ线片，行关节间隙的测量，作统计学ｔ检验。结果：治疗后关节前、后间隙线性测量及面积测量改变值，与治疗前相比有显著性差异（Ｐ＜０．０１），线性百分比为１３．３４±２．０５％，关节后、前间隙面积之比，较治疗前有显著性变化（Ｐ＜０．０５）。结论：前方牵引矫治器通过引导下颌姿势位后移，改善上、下颌骨矢状关系，使髁突后移并稳定于正常关节位，恢复了■、肌肉、关节的协调关系。
Objective: To observe the effects of correction of class Ⅲ malocclusion with anterior traction appliance on temporal mandibular joint. Methods: 19 cases of class malocclusion complicated with TMD in mixed dentition were treated with anterior traction appliances. Bilateral Xray films of Schuller's position were taken before and after treatment respectively. The TMJ spaces were measured and statistically analysed. Results: The linear distances and areas of TMJ spaces show significant difference before and after treatment (P<0.05). The ration between anterior TMJ spaces area and posterior TMJ spaces area also shows significant differences compar to pretreatment. Conclusions: Anterior traction appliance makes the condyle retruded position stay in normal position stably. It can also recover the harmony relatiorship among cclusion, muscles and TMJ.
