摘要
目的 探讨鼻内窥镜手术治疗慢性鼻窦炎的疗效及影响因素。方法 对 1999～ 2 0 0 2年间完成随访的 180 0例慢性鼻窦炎内窥镜手术患者进行临床分析。按照海口标准Ⅰ型患者 2 82例 ,Ⅱ型 10 6 4例 ,Ⅲ型 4 5 4例 ;在局麻下完成手术 16 94例 ,全麻下完成手术 10 6例。术后随访 3～ 14个月 ,平均 5 .5个月。结果 治愈率Ⅰ型 94 .0 % ,Ⅱ型 82 .2 % ,Ⅲ型 77.3% ,三型之间相差非常显著 (P <0 .0 1) ,出现手术并发症者 74例 (4 .1% )。结论 鼻内窥镜手术是治疗慢性鼻窦炎的有效方法 ,疗效与临床分型密切相关。
Objective To evaluate the effect and influence factor of endoscopic sinus surgery (ESS) on chronic sinusitis. Method One thousand eight hundred cases with chronic sinusitis undergone ESS from 1999 to 2002 were appraised.According to the standard criteria of Chinese ENT Association (Haikou,1997),there were 282 cases of type Ⅰ,1 064 cases of type Ⅱ and 454 cases of type Ⅲ,and 1 694 cases underwent ESS under local anesthesia and 106 cases under whole body anasthesia.Long-term follow-up was performed for 3 to 14 months (mean 5.5 months).Results The cure rate was 94.0% for type Ⅰ,82.2% for type Ⅱ and 77.3% for type Ⅲ,There was significant difference in cure rate among the three type (P<0.01),and complication of ESS in all cases occurred in 74 (4.1%).Conclusion ESS is efficacy in the treatment of chronic sinusitis,and the cure result is directly related to the type of the disease.
