摘要
20 0 0年 4月～ 2 0 0 2年 8月 ,对 1例右肾下极错构瘤和 2例右肾上盏憩室并结石患者分别行腹腔镜肾部分切除术。先游离患肾 ,显露肾动 静脉及输尿管 ,用橡皮带阻断患肾血流 ,行肾部分切除术 ;再用氩气刀处理肾脏创面 凝固止血 ;用 2 0可吸收线作“8”字缝合 ,关闭肾切面 ,恢复肾脏血供 ,妥善固定肾脏 ,关闭戳口。 3例手术均获成功 ,无中转开放。手术时间平均 4 .1(3～ 5 )h ;术中出血平均4 0 0 (15 0～ 6 5 0 )ml;肾蒂阻断时间为 30min。术后 3月复查CT ,患肾功能良好 ,无肿瘤和结石复发。
To report initial experience of laparoscopic partial nephrectomy. From April 2000 to August 2002, 3 patients underwent laparoscopic partial nephrectomy. Abdominal cavity was first created. Open the lateral peritoneum and enter the post peritoneal space. After mobilizing the whole kidney with harmonic scalpel and dissecting the pedicle of kidney, a homemade rubber strip was used to control the renal blood supply. Within 30 minutes, incision of the partial kidney, hemastasis and closing of the renal incision were completed. While blood supply to the kidney recovered, bleeding of the renal incison was checked out and fixation of the kidney was accomplished. All the operations were successful. The averaged operation time was 4.1(3～5)h;mean blood loss was 400 ml;mean time for ischamic kidney was 30 minutes. All cases were followed up for 3 months and CT screening showed the kidneys functioned well. Laparoscopic partial nephrectomy is a miniinvasive surgery and applicable to the diseases in the poles of kidney.
