摘要
目的 :总结成人继发孔型房间隔缺损 (房缺 )外科手术治疗的经验。方法 :1993年 10月至 2 0 0 3年 3月 ,37例 2 0岁以上继发孔型房间隔缺损病人 ,年龄 2 0～ 5 7岁 ,平均 (32 5± 4 6 )岁。合并三尖瓣关闭不全 18例 ,合并二尖瓣关闭不全 6例 ,房颤 5例 ,房扑 2例 ,房缺修补采用直接缝合和心包片或涤纶片修补 ,同期行三尖瓣成形术 15例 ,其中 8例行DeVega成形术 ,7例行Kay成形术 ,二尖瓣成形术 4例 ,行二尖瓣置换术 2例。结果 :全组术后无死亡 ,均痊愈出院 ,心功能均有明显改善。结论 :对合并症的同期手术治疗是手术成功的关键 。
Objective:To review the clinical experience in surgical treatment of isolated secundum atrial septal defect in dults.Methods:37 patients over twenty years old underwent surgical repair for secundum atrial septal defect from October 1993 to March 2003.The male patients were 15,female 22,age 20～57 years(average 32 5±4 6 years).Concomitant cardiac anomalys,included moderate to severe tricuspid regurgitation in 18 patients,mitral valve regurgitation in 6,atrial fibrillation in 5 and atrial flutter in 2.All of the patients underwent direct suture or Dacron and pericardia patch.15 patients underwent either De Vega or Kay tricuspid annuloplasty for TVR,and 4 mitral valve repair and 2 mitral valve replacement.Result:All patients survived after surgery.Cardiac function were improved in most of the patients after operation.Conclusion:Proper surgical management of the co existing cardiac anomalys is an imortant facter influencing surgical outcome.Age is not an absolate contradiction to repair of atrial septal defect.
