摘要
目的 评价高度近视白内障超声乳化及人工晶状体植入的效果。方法 13 7例 (174眼 )行巩膜或角膜隧道切口超声乳化 ,通过 5 5mm或 3 2mm切口植入人工晶状体。结果 术后裸眼视力≥ 0 5者第 1天占 51 1% ,1周占 70 2 %。术后 1周、1月和 3月视力≥ 1 0者 ,分别为 2 2 5%、3 8 1%、3 9 7%。术后 1周、1月和 3月角膜散光分别为 (1 76± 1 64)D、(1 15±1 58)D和 (1 91± 1 52 )D。术中虹膜损伤 15眼占 8 6% ,后囊破裂 14眼占 8 0 % ,玻璃体脱出 12眼占 6 8%。术后第 1天角膜水肿 3 1眼 ,出现渗出物 12眼 ,后囊浑浊 19眼 ,脉脱和网脱各 2眼。结论 高度近视白内障超声乳化人工晶状体植入术具有视力恢复快 ,角膜屈光状态稳定 。
Objective To observe the results of phacoemulsification an d intraocular lens(IOL)implantation in high myopia with cataract Methods 137 cases(174 eyes)received phacoemulsification through scleral or corneal tunnel,IOL was implanted through 5 5mm or 3 2mm incision Results Postoperative uncorrected visual acuity was 0 5 or bet te r in 51 1% patients at the first day and 70 2% at one week Visual acuity was 1 0 or better in 22 5%,38 1%,39 7% at 1 week,1 month,3 months respectively,and c orn eal astigmatism was (1 76±1 64)D,(1 15±1 58)D,(1 91±1 52)D respectively Intra oper ative complications included iris damage in 15 eyes(8 6%),posterior capsule rup t ure in 14 eyes(8 0%),vitreous extrusion in 21 eyes (6 8%) One day after opera tio n,there were corneal edema in 31 eyes,exudation in 12 eyes,posterior capusule op acity in 19 eyes,choriodal detachment in 2 eyes and retinal detachment in 2 eyes Conclusion Phacoemulsification and IOL implantation for high my opia with cataract has the advantages of fast vision recovery,corneal refraction stable,no serious complications and so on
