摘要
目的 探讨梗阻性大肠癌的外科治疗。方法 分析 1999～ 2 0 0 2年间 32例梗阻大肠癌的外科治疗资料。结果 32例梗阻性大肠癌 ,Ⅰ期右半结肠切除 6例 ;Ⅰ期横结肠切除 3例 ;Ⅰ期左半结肠切除 3例 ;Ⅰ期左半结肠切除 ,hartmann造口术 18例 ;肿瘤无法切除行乙状结肠造口 2例 ;Ⅱ期吻合 10例 ,病人放弃Ⅱ期吻合 8例。术后并发症发生率 12 8% (4 / 32 ) ,围手术期病死亡 0 0 0 % (0 / 32 )。结论 合理选择梗阻性大肠癌的手术方法 ,加强围手术期处理是减少术后并发症 。
AIM To investigate surgical treatment of obstructive carcinoma of colon. METHODS The 32 clinical documents, which are of surgical treatment of obstructive carcinoma of colon from 1992 to 2002, has been analysed. RESULTS In 32 cases, there were 6 patients trealed with right hemicolectomy (primary suture), 3 patients received resection of transverse colon (primary suture), 18 patients given left hemicolectomy plus Hartmann's fistulization, 2 patients not moved tumor but only given sigmoidostomy, 10 patients received secondary suture and 8 patients abandoned secondary suture themselves. In those cases, the incidence of postoperative complication is 12 5% (4/32) and the mortality is 00 0% (0·32). CONCLUSION Selecting reasonably operative method of obstructive carcinoma of colon and enhancing perioperative treatment can obtain not only reducing postoperative complication, but also preventing postoperative anastomotic leakage.
