摘要
目的 :总结坚强内固定技术在正颌手术中应用的经验。方法 :对 4 1例接受正颌手术的患者采用不同形状钛板行坚强内固定 ,其中下颌升支矢截骨术 (sagittalsplitruamsosteotomy) 2 0例 ,颏成形术 (genioplasty) 7例 ,LeFortⅠ截骨术 5例 ,上颌前份截骨术 (anteriormaxillayosteotomy) 5例 ,双颌手术 (bimaxillarysurgery) 4例。按以下程序进行操作 :先恢复预期咬合关系并暂行颌间栓结 ,然后对截开骨段进行坚强内固定 ,最后拆除暂行的颌间栓结 ,同时检查咬合关系。结果 :采用坚强内固定的患者颌间栓结时间较传统固定方法明显缩短 ,经过术后 1个月、3个月和 6个月随诊复查 ,发现所有截骨段均愈合良好 ,患者下颌运动功能在较短时间内恢复到术前状态 ,所有患者均未诉有不适感。结论 :坚强内固定技术是正颌外科中固定截骨段最好的方法之一 ,它可以加速骨段间的愈合速度 ,减少颌间栓结时间 ,便于患者营养的摄取和保持口腔卫生 。
Objective:The purpose of this study is to evaluate the way of the rigid internal fixation in orthognathic surgery.Methods:According to the site of osteotomy ,different type Ti-platas were used in 41cases of orthognathic surgery (20 cases were sagittal split ruams osteotomy ,5 cases were LeFortⅠosteotomy ,5 cases were anterior maxillary osteotomy ,7 cases were genioplasty,4 cases were bimaxillary surgery).In terms of procedure,normal bite relation was first recovered and intermaxillary fixation,then rigid internal fixation applied between the segments,at last the intermaxillary tixation removed and the bite relation examed.Results:Clinical and x-ray examination were made following one ,three and six months.It was found that the bone segments were successfully union.The mandible motive function of all patients got closer to the pre-operatire condition.Conclusions:The rigid internal fixation is one of the best way of fixation bone segments in orthognathic surgery .It has many advantages including more rapid bone healing and short time of intermaxillary fixation, favourable recovery of mandibular function and improved airway management.
