摘要
目的探讨短期石膏制动对跟骨骨折术后踝关节功能恢复的影响。方法选取该院2014年3月—2018年8月治疗的20例跟骨骨折术后非石膏固定组8例和石膏固定组12例患者的踝关节功能恢复疗效病例资料进行分析,非石膏固定组按照常规处理,石膏固定组术后除按照常规处理外还应用短腿石膏将患肢踝关节中立足轻度外翻位固定,固定3周左右,观察两组术后踝关节功能情况对比。踝关节功能末次随访参照AOFAS踝-后足评分系统(美国足与踝关节协会评分系统进行评分)。结果两组伤口愈合良好。术后3周拆线,20例均获随访,骨折全部愈合,石膏固定组关节疼痛、日常活动以及AOFAS评分总分都大于非石膏固定组,差异有统计学意义(P<0.05)。结论石膏固定组较非石膏固定组有利于术后跟腓韧带、跟距间韧带的修复,增强跟骨骨折术后踝关节稳定性以供大家参考,但还需要增加临床病例及随访时间来进一步验证。
Objective To investigate the effect of short-term gypsum immobilization on the functional recovery of ankle after calcaneal fracture. Methods Twenty patients with calcaneal fracture treated in our hospital from March 2014 to August 2018 were selected to analyze the curative effect of functional recovery of ankle joint in 8 cases of non-gypsum fixation group and 12 cases of gypsum fixation group. The patients in non-gypsum fixation group were treated according to routine treatment. In the gypsum fixation group, in addition to routine treatment, short leg plaster was used to fix the ankle joint based on mild valgus for about 3 weeks. The function of ankle joint was compared between the two groups.The last follow-up of ankle function was based on the AOFAS ankle-hind foot scoring system(American foot and ankle Association scoring system). Results The wound healed well in both groups. All the 20 cases were followed up 3 weeks after operation. All the fractures were healed, the joint pain, daily activity and total score of AOFAS in the plaster fixation group were higher than those in the non-gypsum fixation group,the different was statistically significant(P<0.05).Conclusion Plaster fixation group is more beneficial to the repair of calcaneal ligament and intercalcaneal ligament than that of non-plaster fixation group, and can enhance the stability of ankle joint after calcaneal fracture for reference, but it needs to be further verified by increasing clinical cases and follow-up time.
