摘要
目的观察紫杉醇联合顺铂治疗局部晚期非小细胞肺癌患者的临床疗效及安全性。方法将68例局部晚期非小细胞肺癌患者按照随机数字表法分为对照组和治疗组。对照组:采用手术治疗。治疗组:于手术治疗基础上采用紫杉醇,150 mg/m2静脉滴注;顺铂,25 mg/m2静脉滴注辅助化疗治疗。观察2组患者临床疗效和远期疗效(6个月、1、3年生存率)及不良反应、生存质量。结果治疗组近期治疗有效率为85.29%(29/34),明显高于对照组73.53%(25/34)(χ2=25.19,P<0.05);治疗组6个月、1年、3年生存率分别为67.65%(23/34)、52.94%(18/34)、35.29%(12/34),与对照组55.88%(19/34)、41.18%(14/34)、23.53%(8/34)比较,P<0.05。治疗组发生白细胞降低等不良反应率为8.82%(3/34),明显低于对照组17.65%(6/34)(χ2=13.28,P<0.05)。2组患者生存质量评分均明显改善,但治疗组优于对照组(P<0.05)。结论相比单纯手术治疗局部晚期非小细胞肺癌患者,手术联合紫杉醇及顺铂化疗具有更好疗效,且发生不良反应少,可以提高患者远期疗效及生存质量,同时具有较高安全性。
Objective To observe the clinical efficacy and safety of paclitaxel combined with cisplatin in the treatment of locally advanced non-small cell lung cancer.Methods 68 patients with locally advanced non-small cell lung cancer were randomly divided into the control group and the observation group.The control group received the operation treatment; the observation group on the base of the control group received paclitaxel intravenous infusion of 150 mg/m2; cisplatin intravenous infusion of 25 mg/m2.The clinical efficacy,long-term efficacy(6-month,1-,3-year survival rates),adverse reactions and quality of life of the 2groups were observed.Results Short-term effective rate in the observation group was 85.29%(29 /34),which was significantly higher than that of the control group 73.53%(25 /34)(χ2= 25.19,P < 0.05); 6-month,1-,3-year survival rates in the observation group were 67.65%(23 /34),52.94%(18 /34),35.29%(12 /34),and those of the control group were 55.88%(19 /34),41.18%(14 /34),23.53%(8 /34)(P < 0.05); Rate of adverse reactions such as white blood cells in the observation group was8.82%(3 /34),which was significantly lower than that of the control group 17.65%(6 /34)(χ2= 13.28,P < 0.05).Quality of life scores of the 2 groups were significantly improved,but the observation group was better than that of the control group(P <0.05).Conclusion Compared with simple operation,operation combined with paclitaxel and cisplatin for locally advanced nonsmall cell lung cancer has a better curative effects with less adverse reactions,and it can improve long-term curative effects and life quality of patients with high security.
