摘要
目的:研究低分子肝素治疗肾病综合征(NS)继发高凝状态疗效及对纤溶-凝血系统、肾功能的影响。方法:将某院2015年3月~2017年3月间收治的60例NS继发性高凝状态患者纳为研究对象,随机将其分为观察组(泼尼松+低分子肝素,n=30)与对照组(泼尼松,n=30),比较两组治疗后纤溶-凝血系统相关指标与肾功能水平。结果:治疗后,对照组纤溶-凝血系统相关指标:PT、FIB、DD水平较治疗前无显著性差异(P>0.05);观察组PT、APTT较治疗前显著延长,FIB、DD水平显著下降,AT-Ⅲ显著上升(P<0.05),且各指标与对照组治疗后均具有显著性差异(P<0.05);较治疗前比,两组肾功能指标:BUN、Scr水平均显著下降,且观察组两物质水平下降幅度更大(P<0.05)。结论:低分子肝素可有效改善NS继发高凝状态患者纤溶-凝血系统异常状态,提高患者肾功能水平。
Objective: To study the efficacy of low molecular weight heparin(LMWH) on secondary hypercoagulability of nephrotic syndrome(NS) and its effects of fibrinolysis-coagulation system and renal function. Methods: 60 patients with NS secondary hypercoagulability in a hospital from March 2015 to March 2017 were selected and divided into observation group(prednisone and LMWH, n=30) and control group(prednisone, n=30). The related indexes of fibrinolysis-coagulation system and the renal function were compared between the two groups. Results: After treatment, there was no significant difference in the related indexes of fibrinolysis-coagulation system(PT, FIB, DD levels) in control group compared with those before treatment(P>0.05). Levels of PT and APTT in observation group were significantly longer than those before treatment, and levels of FIB and DD were significantly decreased while AT-Ⅲ level was significantly increased(P<0.05), and there were significant differences in indexes levels between observation group and control group after treatment(P<0.05). Compared with before treatment, levels of renal function indexes(BUN, Scr) were significantly decreased in two groups, and levels were decreased more significantly in observation group(P<0.05). Conclusion: LMWH can effectively improve fibrinolysis-coagulation system abnormalities and the renal function of patients with NS secondary hypercoagulability.
