摘要
Barrett食管(BE)是食管下段的鳞状上皮异生为柱状上皮的病理现象,如伴有肠上皮化生,则属于食管腺癌的癌前病变。现代医学对于BE的发病机制尚未完全明确,目前认为BE的发生与食管抗反流屏障障碍、食管酸清除能力下降、食管黏膜防御功能低下及胃排空延迟等多种因素有关。治疗上以应用抑酸剂、促动力药、黏膜保护剂为主要手段,但疗程较长,并且疗效不尽如人意。随着生活环境、饮食结构等的改变,以及胃镜检查技术的发展普及,BE的检出率逐年增高。我国是食管癌的高发区,由于部分BE病例存在一定程度的恶性潜能,对于BE的诊治愈来愈被临床所重视。近年来,对BE的中医药诊治屡见报端。文章介绍了白长川教授应用中医药治疗BE的经验体会,并从现代医学与中医学相契合的角度对BE的发病机理、诊治等方面进行探讨。
Barrett's esophagus( BE) is the pathologic phenomena characterized as columnar epithelium of abnormal stratified squamous epithelium in the lower esophagus,associated as intestinal metaplasia,belonging to the precancerous lesion of esophageal adenocarcinoma. The pathogenic mechanism on this disease is unknown yet in modern medicine. Currently,it is believed that BE is related to the impairment of esophagus on anti- reflux barrier,the decrease of esophageal acid clearance,the lower esophageal mucosal defense and delayed gastric emptying,etc. In treatment,acid inhibitor,the gastro- kinetic agent and the agent for protecting mucous membrane are used. But the duration of treatment is long and the efficacy is not so satisfactory.With the changes in living environment and dietary structure,as well as the development of gastroscopic technology,the detective rate of BE is increasing yearly. The incidence of esophageal cancer is high in China. Because of malignant possibility of BE,the diagnosis and treatment of BE has drawn more and more attention in clinical practice. In recent years,there are more reports on TCM treatment for BE. In the paper,the experiences of Prof. Bai Changchuan were introduced in TCM treatment for BE and the discussion was carried on the pathogenic mechanism of BE in view of the integration of modern medicine and Chinese medicine.
