摘要
报告540例胆囊炎和胆石症病例经手术验证的 B 型超声诊断结果:它诊断胆囊炎和胆囊结石的符合率为97.6%,高于胆囊造影术之87.9%,两者间的差别非常显著(P<0.01);在 B 型超声检查中,上述胆囊组的符合率高于胆总管结石组的52.5%,两组间的差别也非常显著(P<0.01)。此外,还根据其中500例胆囊炎和胆囊结石的声象图表现进行了分型和探讨。
[Ⅰ]From Jan 1984 to Oct, 1990,in a group of 540 inpatients aged 16—84 Yrs,using the B-mode ultrasonography(B-US),(1)533 cases were diagnosed as cholecystitis pl- us cholelithiasis,33 of the latter be- ing complicated with choledocholith- iasis too,Upon operative surgery (os),it was verified that the tally- with rate(TWR)was 97.6%;(2)7 ca- ses were diagnosed as choledocholit- hiasis,together with the 33 cases mentionecl above,upon OS,it was ve- rifled that the TWR was only 52.5%. [Ⅱ]During the above 1984～1990 per- iod,in another group of 66 inpatie- nts with tentative clinical diagnosis of cholecystitis plus cholelithiasis, by Using cholecystography(CHCYS), all of their diagnosis were confirm ed successively;Yet upon OS,it was verified that the TWR was only 87.8%. Comparing the TWR 97.6% of group Ⅰ(1)with TWR 87.8% of group Ⅱ, their difference is of very marked statistical significance(P<0.01),indica- ting that B-US is superior to CHC- YS in diagnosing cholecystitis plus cholelithiasis,While within group Ⅰ, comparing the above TWR 97.6% with the low TWR 52.5% of the partly over lapping(2),their difference is also of very marked statistical differe- nce,indicating that up to the present, in diagnosing choledocholithiasis(wh- ether or not complicated with cholecy- stitis plus cholelithiasis),B-US is yet unsatisfactory & unreliable.
