摘要
目的观察补肺化痰益肾祛瘀方治疗慢性阻塞性肺疾病稳定期的临床疗效及其对血清D-二聚体的影响。方法将80例慢性阻塞性肺疾病稳定期患者随机分为治疗组与对照组,每组40例。对照组予西医常规治疗,治疗组予中药补肺化痰益肾祛瘀方+西医常规治疗。观察比较中医证候积分、肺功能及血清D-二聚体水平的变化情况。结果 (1)最终完成试验者75例,治疗组39例、对照组36例。(2)治疗前后组内比较,两组中医证候总积分均明显降低(P<0.05);组间治疗前后差值比较,治疗组明显多于对照组(P<0.05)。(3)治疗前后组内比较,两组FVC、FEV1(%)水平差异均有统计学意义(P<0.05),而FEV1水平差异无统计学意义(P>0.05);组间治疗后比较,肺功能各指标差异无统计学意义(P>0.05)。(4)治疗前后组内比较,治疗组D-D水平明显降低(P<0.05),而对照组DD水平差异无统计学意义(P>0.05);组间治疗前后差值比较,治疗组D-D水平降低程度更甚于对照组(P<0.05)。结论补肺化痰益肾祛瘀方可明显减轻COPD缓解期患者的临床症状,降低血清D-二聚体水平,从而减轻COPD的慢性炎症状态、改善预后。
Objective To observe the clinical efficacy of Bufei Huatan Yishen Quyu Recipe in the treatment of chronic obstructive pulmonary disease( COPD) in stable stage and its effects on serum level of D-Dimer. Methods 80 COPD patients in stable stage were randomly divided into the treatment group and control group,40 cases in each group. The control group was treated with the conventional therapy of western medicine,and the treatment group was treated with Bufei Huatan Yishen Quyu Recipe based on the treatment for the control group. The changes on Chinese medical syndrome score,pulmonary function indexes and serum level of D-Dimer( D-D) were observed and compared. Results(1)Finally 75 cases completed the trial,among 39 cases in the treatment group and 36 cases in the control group.(2)Compared with treatment before,the Chinese medical syndrome score was obviously decreased in both groups after treatment( P < 0.05). The difference of Chinese medical syndrome score between treatment before and after in the treatment group was greater than that in the control group( P<0.05).(3)In both groups,there were statistically significant differences on the levels of FVC and FEV1( %) between treatment before and after( P< 0.05),but there was no statistically significant difference on the level of FEV1 between treatment before and after( P>0.05). There were no statistically significant differences on the levels of pulmonary function indexes between the two groups( P>0.05).(4)Compared with treatment before,the D-D level was obviously decreased in the treatment group after treatment( P<0.05),but there was no statistically significant difference on the D-D level in the control group between treatment before and after( P > 0.05).The difference of D-D level between treatment before and after in the treatment group was greater than that in the control group( P < 0.05).
