摘要
目的观察龟龄集胶囊联合显微精索静脉高位结扎术治疗精索静脉曲张致不育症的临床疗效。方法将93例精索静脉曲张致不育症患者随机分为治疗组(47例)与对照组(46例)。两组均行显微精索静脉高位结扎术治疗,治疗组术后加服龟龄集胶囊治疗、疗程为6个月。术后随访1年,观察比较手术前及术后6个月两组精液质量(精子密度、精子总活力、精子前向运动率)、性功能[国际早泄评分指数-5(CIPE-5)及勃起功能指数-5(IIEF-5)]、精浆及血清缺氧诱导因子1α(HIF-1α)水平,比较随访1年期间两组配偶受孕情况。结果 (1)组间术后6个月比较,精子密度、精子总活力、精子前向运动率差异有统计学意义,治疗组对精液质量的改善明显优于对照组(P<0.05)。(2)组间术后6个月比较,CIPE-5、IIEF-5指数差异有统计学意义,治疗组指数明显高于对照组(P<0.05)。(3)组间术后6个月比较,精浆HIF-1α、血清HIF-1α水平差异有统计学意义,治疗组明显低于对照组(P<0.05)。(4)随访期间,治疗组、对照组受孕率分别为65.96%、45.65%;组间受孕率比较,差异有统计学意义(P<0.05)。结论龟龄集胶囊联合显微精索静脉高位结扎术治疗精索静脉曲张致不育症患者,疗效显著,可提高患者精液质量及性功能,有利于配偶受孕。
Objective To observe the clinical efficacy of Guilingji Capsule combined with microscopical high ligation of spermatic vein on postoperative semen quality and conception rate in varicocele-induced infertile patients. Methods 93 varicocele-induced infertile patients were randomly divided into the treatment group( n = 47) and control group( n = 46). Both groups were treated with microscopical high ligation of spermatic vein,and the treatment group was additionally treated with Guilingji Capsule for continuous 6 months. The patients were followed-up for one year after operation. The semen quality( sperm density,total sperm activity,sperm forward motility rate),sexual function [Chinese index of sexual function for premature ejaculation-5( CIPE-5) and international index of erectile function-5( IIEF-5) ],seminal plasma and serum levels of hypoxia inducible factor-1α( HIF-1α) in the two groups were compared before operation and 6 months after operation. During the follow-up,the conception rates in both groups were analyzed. Results(1)6 months after operation,there were statistically significant differences on sperm density,total sperm activity and sperm forward motility between the two groups,and the improvement on the semen quality in the treatment group was better than that in the control group( P<0.05).(2)6 months after operation,there were statistically significant differences on CIPE-5 and IIEF-5 scores between the two groups,and the scores of the treatment group were higher than those of the control group( P<0.05).(3)6 months after operation,there were statistically significant differences on the levels of HIF-1α in seminal plasma and serum between the two groups,and the levels of the treatment group were lower than those of the control group( P<0.05).(4)During the follow-up,the conception rates were 65.96% in the treatment group and 45.65% in the control group,there was statistically significant difference on the conception rate between the two groups( P < 0.05).
