摘要
目的观察四蚕加味方治疗慢性肾小球肾炎的临床疗效。方法将82例慢性肾小球肾炎患者随机分为治疗组41例和对照组41例。两组患者均给予西医基础治疗,同时,治疗组患者给予四蚕加味方联合参芪地黄汤治疗,对照组患者给予参芪地黄汤治疗,两组疗程均为2个月。评价两组患者的临床疗效,检测并比较患者的血压、24 h尿蛋白定量、血尿及肾功能(包括SCr、eGFR)指标,检测所有患者的血IgA、IgM、IgG、补体C3水平及T细胞亚群(CD4^+、CD8^+)比例。结果治疗后,治疗组临床总有效率为90.0%,对照组为57.5%,治疗组的临床疗效优于对照组(P<0.05)。治疗后,治疗组患者的血尿、24 h尿蛋白定量水平均明显降低(P<0.05),对照组患者的24 h尿蛋白定量水平亦明显降低(P<0.05),且治疗组患者的24 h尿蛋白定量水平低于对照组(P<0.05)。治疗后,治疗组患者的IgA、CD8^+水平明显降低(P<0.05),IgM、补体C3、CD4^+/CD8^+水平明显升高(P<0.05);对照组患者仅补体C3水平明显升高(P<0.05);且治疗组患者的补体C3、CD4^+/CD8^+水平高于对照组(P<0.05),CD8^+水平低于对照组(P<0.05)。结论四蚕加味方能显著改善慢性肾小球肾炎患者的临床症状,减少尿蛋白,调节体液免疫和细胞免疫功能,从而体现了"从风论治"慢性肾小球肾炎的临床应用价值。
Objective To observe the clinical efficacy of Modified Sican Formula in the treatment of chronic glomerulonephritis. Methods 82 chronic glomerulonephritis patients were randomly divided into the treatment group and control group,41 cases in each group. Both groups were treated with basic therapy of western medicine. The treatment group was treated with Modified Sican Formula and Shenqi Dihuang Decoction,and the control group was treated with Shenqi Dihuang Decoction,with a course of 2 months. The clinical efficacy was evaluated in both groups. The blood pressure,24 h urine protein quantification,hematuria and renal function indexes( including Scr and eGFR) were detected and compared,the serum levels of IgA,IgM,IgG,complement C3 and the proportion of T cell subsets( CD4^+,CD8^+ ) were measured in all patients. Results The clinical total effective rates was 90.0% in the treatment group and 57.5% in the control group,the clinical efficacy of the treatment group was better than that of the control group( P < 0.05). After treatment,the levels of hematuria and 24 h urine protein quantification were decreased in the treatment group( P<0.05),the level of 24 h urine protein quantification was also decreased in the control group( P<0.05),and the level of 24 h urine protein quantification in the treatment group was lower than that in the control group( P<0.05). After treatment,the levels of IgA and CD8^+ were decreased in the treatment group( P<0.05),and the levels of IgM,complement C3 and ratio of CD4^+/CD8^+ were increased( P<0.05). The level of C3 was also increased in the control group( P<0.05). The levels of complement C3 and ratio of CD4^+ /CD8^+ in the treatment group were higher than those in the control group( P<0.05),and the level of CD8^+ in the treatment group was lower than that in the control group( P < 0.05).
