摘要
目的:探讨川芎平喘合剂治疗哮喘的临床疗效及对患者血清Th1/Th2及相关转录调节因子T-bet/GATA-3平衡的影响。方法:选取中医辨证为风哮夹瘀型且符合慢性持续期支气管哮喘诊断的患者,随机分为对照组和治疗组。对照组采用西医常规治疗(吸入沙美特罗/氟替卡松吸入剂),治疗组在对照组基础上加服川芎平喘合剂,疗程均为1个月。监测治疗前后两组中医证候积分、哮喘控制测试(ACT)评分、肺功能,检测外周血γ-干扰素(IFN-γ)、白介素-4(IL-4)水平及转录因子T-bet、结合蛋白-3(GATA-3)的mRNA表达变化,评价临床疗效。结果:两组中医证候积分均较治疗前显著下降(P<0.01),ACT评分、肺功能指标[1秒用力呼气容积占预计值%(FEV1%)、最大呼气峰流量占预计值%(PEF%)]均较治疗前明显升高(P<0.01),且治疗组的改善作用均优于对照组(P<0.01或P<0.05);两组血清IFN-γ、T-bet mRNA均明显升高(P<0.01),血清IL-4、GATA-3 mRNA均明显降低(P<0.01),且治疗组对IFN-γ/IL-4及T-bet/GATA-3的调节作用均显著优于对照组(P<0.05)。治疗组总有效率为96.7%,对照组为80.0%,两组总体疗效差异有统计学意义(P<0.05)。结论:川芎平喘合剂对哮喘的疗效确切,且可能与调节患者Th1/Th2及相关转录调节因子T-bet/GATA-3的平衡有关。
Objective: To investigate the clinical efficacy of Chuanxiong Pingchuan Mixture in the treatment of asthma and the effects on the balance of serum Th1/Th2 and related transcriptional regulator T-bet/GATA-3. Methods: The patients who diagnosed as syndrome of wind wheezing with blood stasis in TCM syndrome differentiation and chronic persistent bronchial asthma were randomly divided into the control group and treatment group. The control group was treated with routine treatment of western medicine( inhaling salmeterol/fluticasone inhalation),the treatment group was treated with Chuanxiong Pingchuan Mixture based on the treatment for the control group,with a course of one month. Before and after treatment,the TCM syndrome score,asthma control test( ACT) score and lung function indexes were monitored in both groups,and the changes on interferon-γ( IFN-γ),interleukin-4( IL-4),transcription factor T-bet mRNA and binding protein-3( GATA-3) mRNA in peripheral blood were detected,and the clinical efficacy was evaluated. Results: After treatment,the TCM syndrome score was significantly decreased in both groups( P < 0.01),and the ACT score,pulmonary function index( FEV1% and PEF%) were significantly increased( P<0.01),the improvement in the treatment group was better than that in the control group( P<0.01 or P<0.05). The serum levels of IFN-γ and T-bet mRNA were significantly increased in both groups( P<0.01),and the serum levels of IL-4 and GATA-3 mRNA were significantly decreased( P<0.01),and the regulation on IFN-γ/IL-4 and T-bet/GATA-3 in the treatment group were significantly better than those in the control group( P<0.05). The total effective rates were 96.7% in the treatment group and 80.0% in the control group. There was statistically significant difference on the total efficacy between the two groups( P< 0.05).Conclusion: Chuanxiong Pingchuan Mixture shows definite efficacy in the treatment of asthma,and it may be related to the regulation on balance of Th1/Th2 and related transcriptional regulator T-bet/GATA-3
