摘要
目的 :研究 5种氟喹诺酮类抗菌药物 (FQNS)对 2 0 0 1- 2 0 0 3年成都地区 3家教学医院分离的 2 6 6株菌的体外抗菌活性。方法 :采用琼脂二倍稀释法测定了环丙沙星、氧氟沙星、左氧氟沙星、司帕沙星和加替沙星对2 6 6株分离菌的最低抑菌浓度。结果 :加替沙星和司帕沙星对 2 6 6株临床分离菌的MIC50 和MIC90 均为 0 2 5和 8mg/L ,低于环丙沙星 (0 5和 16mg/L)、氧氟沙星 (2和 16mg/L)、左氧氟沙星 (0 5和 8mg/L)。 5种FQNS对葡萄球菌具有很好的抗菌活性 ,对肺炎链球菌全部敏感 ,且新型FQNS的MIC50 和MIC90 是传统FQNS的 1/4~ 1/16 ,但是FQNS对肠球菌的抗菌作用不理想。 5种FQNS对流感嗜血杆菌具很好的抗菌活性 ;对大肠埃希菌、克雷伯菌属、枸橼酸杆菌属具有较强的抗菌活性 ,且新型FQNS略优于或相似传统FQNS ;但是FQNS对肠杆菌属的抗菌作用不理想 ,新型FQNS优于传统FQNS ,产气肠杆菌比阴沟肠杆菌敏感得多 ;而变形杆菌和沙雷菌属耐药率高。对于非发酵菌中铜绿假单胞菌新型FQNS和传统FQNS相似 ,而对不动杆菌属和嗜麦芽窄食单胞菌的MIC50 和MIC90 是传统FQNS的 1/8。结论 :加替沙星和司帕沙星等新型FQNS对于革兰阳性菌和革兰阴性菌都具有很强的体外抗菌活性 。
Objective:To survey in vitro activity of five fluoroquinolones (FQNS)clinical isolates from three teaching hospital in Chengdu.Methods:Minimal inhibitory concentrations(MIS s)of ciprofloxacin,ofloxacin,levofloxacin,sparfloxacin and gatifloxacin were measured using two-fold agar dilution method.Results:Five FQNS exhibited the highest activities against gram-positivie cocci,and MIS 50 and MIS 90 were 1/4~1/16 of traditional FQNS,but the activity against enterococci was not good.All Heamophilus spp.isolates were fully susceptible to all FQNS tested.Five FQNS had equal activities against Enterobacteriaceae.Five FQNS showed the highest antimicrobial activeties against Pseudomonas spp,while gatifloxacin and sparfloxacin were more active against Acinetobater spp.and Stenotrophomonas maltoophilia.Conclusions:Gatifloxacin and sparfloxacin showed highest activities against gram-positive cocci and gram-negative bacilli,and the activities against gram-positive cocci was more than that of gram-negative baciili.It also exhibited good activeties to Citrobacter spp,Pseudomonas spp,Acinetobater spp,and Stenotrophomonas maltophilia which were higher resistant strains in clinic.
