简易腹部加压装置技术对减小呼吸运动在原发性肝癌放射治疗摆位误差的影响 被引量:7

摘要 目的研究南通大学附属南通肿瘤医院简易腹部加压装置对减小呼吸运动在原发性肝癌放射治疗时摆位误差影响。方法选择2016年7月至2018年6月南通大学附属南通肿瘤医院原发性肝癌患者60例,其中男性56例,女性4例;年龄38~68岁,中位年龄53岁。随机分为试验组与对照组,试验组32例使用腹部加压固定技术,对照组28例使用真空垫固定技术。通过模拟机透视下测量两组肝上缘头脚方向移动幅度及锥形束CT(CBCT)扫描得到的图像与计划系统传输图像进行骨性加灰度匹配的摆位误差测量分析。结果试验组、对照组肝上缘头脚方向移动幅度分别为(0.36±0.11) cm、(1.18±0.34) cm。试验组、对照组三维方向(X为左右方向、Y为前后方向、Z为背腹方向)位移误差分别为(0.19±0.07) cm、(0.34±0.08) cm、(0.17±0.07) cm和(0.19±0.58) cm、(1.52±0.28) cm、(0.21±0.56) cm。结论该技术能够有效减小呼吸运动对原发性肝癌放射治疗摆位误差的影响,体位重复性好,有效控制原发性肝癌放射治疗过程中肝上缘头脚方向的移动幅度。 Objective To study the effect of simple abdominal compression device on reducing setup error of respiratory movement in radiotherapy for primary liver cancer.Methods From July 2016 to June 2018,60 patients with primary liver cancer were enrolled,which included 56 males and 4 females,aged 38-68 years old with median age of 53 years old.All of them were randomly divided into experimental group(n=32,with abdominal compression fixation)and control group(n=28,with vacuum pad fixing technology).The amplitude of top-bottom movement by fluoroscopy and cone beam CT(CBCT)scan image were measured,and transfer image with planning bone and gray scale matching setup error were analyzed between 2 groups.Results The top-bottom movement of liver upper margin in experimental group and control group was(0.36±0.11)cm,(1.18±0.34)cm,respectively.The three-dimensional(X was left-right direction,Y was anterior-posterior direction,and Z was back-abdominal direction)setup errors of experimental group and control group were(0.19±0.07)cm,(0.34±0.08)cm,(0.17±0.07)cm and(0.19±0.58)cm,(1.52±0.28)cm,(0.21±0.56)cm,respectively.Conclusion It is demonstrated that this technique can effectively reduce the interfere of respiratory movement on setup error of radiotherapy for primary liver cancer,and effectively control movement of top-bottom direction of the upper edge of the liver during radiotherapy for primary liver cancer.
