严重脓毒症早期血清乳酸、降钙素原、脑钠肽联合监测对患者预后的判断价值 被引量:10

摘要 目的探讨严重脓毒症早期血清乳酸(Lac)、降钙素原(PCT)、脑钠肽(BNP)联合监测对患者预后的判断价值。方法 68例严重脓毒症患者,包括存活组35例、死亡组33例。监测两组入住ICU 72 h内血清Lac、PCT、BNP变化,采用受试者工作特征(ROC)曲线分析各指标下降幅度对预测脓毒症患者28 d病死率的灵敏度和特异度。结果死亡组入住ICU 72 h内血清Lac、PCT、BNP下降幅度均低于存活组,P均<0.05。Lac、PCT、BNP单指标与3项指标下降幅度对预测患者28 d病死率的敏感度分别为82.9%、85.7%、80.0%、91.4%,特异度分别为78.8%、72.7%、51.5%、90.9%。结论严重脓毒症患者早期血清Lac、PCT、BNP联合监测有助于判断患者预后。
