摘要
目的:比较同一患者不同采血量血气结果,以确定危重症患者适宜的动脉采血量,从而减少医源性贫血的发生。方法:选择监护室因呼吸衰竭致机械通气留置桡动脉置管患者30例,经桡动脉穿刺置管同时留取0.5、0.8、1.2、1.6ml 4种不同血量的血气标本,采集完毕立即按照标本的采集顺序进行血气分析。比较各组凝血标本数、血气包堵塞次数,比较各组pH、PO_2、PCO_2、HCO3-、BE、SPO_2、血红蛋白、Ca2+, K~+, Na~+, Cl~-及乳酸水平变化。结果:不同采血量对血气的pH、PO_2、PCO_2、HC03、BE、SPO_2、K~+、Na~+、Cl~-、Ca2+、乳酸均无明显影响(P>0.05),而0.5ml采血量测定的血红蛋白量较其他采血量明显减少(P<0.05),其余各采血量血红蛋白的测定结果无明显差异(P>0.05)。结论:0.5ml动脉血量可满足临床血气分析的要求,值得推广。
Objective:To confirm the appropriate volume of arterial blood collected from critically ill patients and decrease the iatrogenic anemia by comparing the blood gas results of different volume of arterial blood. Methods:30 patients with respiratory failure treated with mechanical ventilation were chosen. Different volume of arterial blood including 0.5 ml, 0.8 ml, 1.2 ml and 1.6 ml were collected via radial artery puncture and performed blood gas analysis. Results:Data of blood gas including pH, PO2, PCO2, HCO3-, BE, SPO2, hemoglobin, Ca2+, K+, Na+, Cl-, and lactic acid were compared. Results No significant differences were found in pH, PO2, PCO2, HCO3-, BE, SPO2, levels of K+, Na+, Cl-, Ca2+ and lactic acid between different arterial blood volume(P>0.05). However, level of hemoglobin in arterial blood sample of 0.5 ml was significantly different from other blood samples of different volume(P<0.05). No significant differences of hemoglobin were found between blood samples of 0.8 ml, 1.2 ml and 1.6 ml(P>0.05). Conclusion:0.5 ml of arterial blood can meet the need of arterial blood gas analysis and deserves application in clinic.
