摘要
目的:分析在口腔治疗中女性患者发生体位性低血压的危险因素,为临床患者评估提供理论依据,提高患者的就诊安全。方法:选取2017年6月-2017年9月在我院某科进行口腔治疗的550例女性患者,采用立式水银血压计测量并记录血压,并对体位性低血压发生情况、一般临床资料、牙科焦虑状况以及营养状况进行统计分析。结果:本研究550例女性患者,按照体位性低血压的诊断标准,124例患者可诊断为体位性低血压。单因素分析,年龄、牙科焦虑状况、营养状况对引发体位性低血压有统计学意义(X2值分别为1.335,1.421,1.358;P<0.05)。通过Logistic回归分析,体位性低血压与年龄[OR=0.769,95%CI=(0.406～0.558)]、牙科焦虑状况[OR=0.711,95%CI=(0.357～0.586)]、营养状况[OR=0.784,95%CI=(0.413～0.612)]呈现正相关(P<0.05)。结论:在女性患者口腔治疗中,应重视容易引发体位性低血压发生的危险因素。因此医护人员在口腔门诊中,需要更加注重患者的安全评估,加强对容易发生体位性低血压这类人群的关注和防护。
Objective:To analyze the risk factors of postural hypotension in female patients during oral treatment, so as to provide theoretical basis for clinical assessment and improve the safety of patients. Methods:From June 2017-2017 year in September 550 cases of female patients with oral treatment in our hospital of a branch, the vertical mercury sphygmomanometer to measure and record the blood pressure, and orthostatic hypotension incidence, clinical data, dental anxiety and nutritional status for statistical analysis. Results:In this study, 550 female patients were diagnosed as postural hypotension according to the diagnostic criteria of postural hypotension. Univariate analysis showed that age, dental anxiety and nutritional status had statistical significance in inducing orthostatic hypotension(X2 = 1.335, 1.421, 1.358, P < 0.05). Logistic regression analysis showed that positional hypotension was positively correlated with age [OR=0.769,95%CI=(0.406 to 0.558), dental anxiety [OR=0.711,95%CI=(0.357 to 0.586), and nutritional status [OR=0.784,95%CI=(0.413 to 0.612)](P < 0.05). Conclusion:In the oral treatment of female patients, we should pay more attention to the risk factors that easily lead to the occurrence of postural hypotension. Therefore, in the dental clinic, medical staff need to pay more attention to the safety assessment of patients, and strengthen the attention and protection for people prone to postural hypotension.
