摘要
目的 总结经枕下乙状窦后入路手术切除大型听神经瘤的经验和技巧 ,以提高肿瘤的全切率及面神经的保留率 ,预防并发症。方法 对我院自 1997年 6月至 2 0 0 2年 6月经乙状窦后入路手术切除 4 2例大型听神经瘤 ,进行回顾性分析研究。结果 本组肿瘤全切 38例 (90 .4 % ) ,近全切除 4例 (9.6 % ) ,面神经解剖保留 31例(73.8% )。结论 经枕下乙状窦后入路对大型听神经瘤是一种安全有效的入路 。
Objective\ To summarize experience and techniques of huge acoustic neuroma surgery by suboccipital retrosigmoid transmeatal approach in order to improve total tumors resection and facial nerves reservation and to prevent complications.Methods\ 42 patients of huge acoustic nearoma operated by suboccipital retrosigmoid transmeatal approach from June 1997 to June 2002 in our hospital were studied retrospectively.Results\ Total tumors resectien was achieved in 38 of the 42 patients(90.4%),near total in 4(9.6%).Facial nerves were kept anatomic inact in 31(73.8%) of the patients. Conclusion\ Saboccipital retrosigmoid transmeatal is safe and effective to huge acoustic neuroma surgery.It is critical to know the cerebelcopontine angles' anatomy well and skillful operation for keeping facial nerves anatomic.
