Relationship Between Stem Cell Factor and Gonadotropin on the Development of Ovarian Follicles During Superovulation Stimulation

摘要 目的 :探讨促超排卵周期中干细胞因子 (SCF)与促性腺激素 (FSH和LH)对卵泡发育是否有协同效应。方法 :对 30例进行体外受精 胚胎移植 (IVF ET)的患者 ,给予相同的促超排卵方案 ,采用ELISA和RIA检测患者用药前 ,取卵时血清及卵泡液中SCF、促卵泡激素 (FSH)和黄体生成素 (LH)的水平。按取卵时平均直径≥ 14mm的卵泡数目将卵巢反应分为 3种类型 :即低反应型、中反应型和高反应型。结果 :高、中反应组的卵泡液SCF水平显著高于低反应组 ,差异有非常显著性 (P <0 0 1) ;FSH在高、中反应组卵泡液中水平显著高于低反应组 ,差异有显著性 (P <0 0 5 ) ;3组卵泡液中LH水平不同 ,高、中反应组高于低反应组 ,差异有显著性 (P <0 0 5 ) ;3组中直径≥ 18mm卵泡数目差别很大 ,差异有非常显著性 (P <0 0 1) ;而 3组中发育卵泡 (直径≥ 5mm)总数相近 ,差别无显著性 (P >0 0 5 )。结论 :促超排卵周期中 ,干细胞因子和促性腺激素在促进卵泡发育过程中可能有协同作用。 Objective:To investigate whether stem cell factor (SCF) and gonadotropin (Gn) have synergy during superovulation stimulation in in-vitro fertilization and embryo transfer (IVF-ET) program. Methods:30 cycles of IVF-ET in 30 patients with regular menstrual period were studied. The same superovulation regimen was employed. Enzyme-linked immunosorbent assay (ELISA) and radioimmunoassay (RIA) were used to determine the levels of SCF, follicular-stimulating hormone (FSH) and luteinizing hormone (LH) in follicular fluid (FF) during oocyte pick-up (Opu) day and in serum before and after superovulation. They were divided into three groups:low reactive group,moderate reactive group,and high reactive group according to the number of follicle that diameter≥14 mm on Opu day.Results:FF-SCF levels in high and moderate reactive groups (564±64 ng/L,532±55 ng/L,respectively) were significantly higher than that of low reactive group (352±78 ng/L),P<0.01; FF FSH levels were significantly different in three groups (23.7±5.7 U/L,24.5±8.9 U/L ,and 15.4±4.1 U/L,respectively),P<0.05. FF-LH levels in low reactive group (33.0±7.4 U/L) were lower than those of others two groups (43.5±9.3 U/L,44.2±10.4 U/L,respectively),P<0.05. The number of follicle which diameter exceeded 20 mm was different in three groups (2.6±0.4,4.7±0.5,and 5.3±0.9,respectively),P<0.01,whereas the total number of developing follicles (diametre≥5 mm) was similar,P>0.05. Conclusions:SCF and gonadotropins may have coordinated effect on development of follicles during gonadotropin stimulation in IVF-ET program.
