人工合成聚甘醇碳酸(MAXON)可吸收缝线关节镜下膝关节半月板缝合修复术 被引量:7

摘要 目的 探讨人工合成聚甘醇碳酸 (MAXON)可吸收缝线在关节镜下膝关节半月板缝合术中的治疗效果。 方法半月板损伤 15 3例在镜下应用MAXON可吸收缝线缝合裂伤的半月板 ,其中缝合 2针 119例 ,4针 2 8例 ,6针 4例 ,8针 2例。术后由专科治疗师指导康复训练。 结果  5例术后 3周~ 8周时缝线打结处轻度疼痛 ,未予特殊处理自行恢复。平均随访 2 6月 ,3例关节劳累后出现胀痛 ,1例症状复发 ,2例关节屈曲活动受限 2 0°。其余患者症状消失 ,无关节弹响 ,关节功能良好。Ly sholm评分 ,术前 (5 8± 13)分 ,术后 (93± 7)分 (t=2 6 2 8,P <0 0 1)。 结论 MAXON可吸收缝线关节镜下缝合半月板 ,具有安全、可靠、操作简便等优点。缝线吸收后 ,避免对半月板的制约 ,使愈合的半月板更好地发挥生理和生物力学功能。 Objective To investigate the efficiency of Maxon absorbable sutures in arthroscopic meniscus repair. Methods A total of 153 patients with meniscus injuries underwent arthroscopic repair with Maxon absorbable sutures. The meniscus ruptures were repaired with 2 sutures in 119 patients, with 4 sutures in 28 patients, 6 sutures in 4 patients and 8 in 2 patients. Postoperatively, patients' rehabilitation was under the directions of special therapy technicians. Results Five patients felt a slight pain near the site of knoting 3~8 weeks after the operations, and then freedom from pain was achieved spontaneously without medical treatment.The mean follow-up time was 26 months. A distending pain on operated knee occurred after vigorous activities in 3 patients. Recurrence was seen in 1 patient. Two patients had 20° limitation of knee flexion. All other patients had excellent knee functions, without joint clicking or other symptoms.Pre- and post-operative Lysholm knee scores were (58±13) points and (93±7) points, respectively (t=2.628, P<0.01). Conclusions Application of Maxon absorbable sutures in arthroscopic meniscus repair is safe, reliable and easy to use. Repaired meniscus will play better physiological and biomechanical functions, without the confinement for normal knee flexion after sutures absorbed.
