二尖瓣置换术患者ICAM-1变化及乌司他丁的抑制作用

摘要 将 2 0例择期行二尖瓣置换手术患者随机分为两组 ,观察组应用乌司他丁 (12 0 0 0 U/kg,于麻醉诱导后静注 ) ,对照组不加乌司他丁。分别在麻醉诱导后、体外循环 30分钟、手术结束时、术后 1、2天抽取桡动脉血标本 ,用 EL ISA法测定血清中的细胞间粘附分子 - 1(ICAM- 1)。结果两组血清 ICAM- 1基础值均在正常范围内 ,体外循环开始 30分钟两组 ICAM- 1水平均下降 ,与基础值有显著差异 ,但两组间无显著差异 ;手术结束和术后第 1天 ,对照组血清 ICAM- 1值逐渐上升并超过基础值 ,观察组逐渐下降至基础值 ;手术后第 2天 ,对照组血清 ICAM- 1开始下降但仍超过基础值 ,观察组无变化 ,两组对比有显著差异。认为乌司他丁能明显抑制二尖瓣置换术患者血清 ICAM- 1的表达 ,从而有效降低患者由体外循环引发的炎症反应。 To explore the effects of ulinastatin on ICAM 1 in pa tients undergoi ng mitral valve replacement,20 patients scheduled for elective mitral valve rep lacement were randomized into two groups: ① control (n=10), or ② venous inje ction of ulinastatin (12000U/kg) after anesthesia induction (n=10) Blood sampl e s were drawn from radial artery at baseline, 30 minutes of CPB, end of operation , 1 day after operation and 2 day after operation ICAM 1 in blood serum was m ea sured by ELISA The results showed the institution of CPB in the patients under g oing mitral valve replacement induced up regulation of the serum ICAM 1 Ther e w as no significance in baseline value Samples from the control group demonstrat e d significant increases (P<0 05) in ICAM 1 when compar ed to unlinastatin group a t the same time intervals These results indicate that ulinastatin modulates th e CPB induced up regulation of serum ICAM 1, the important indicator of the sy ste mic inflammatory response to CPB Therefore, these data indicate that ulinastat i n is effective in reducing this inflammatory response, attenuating heart and lun g reperfusion injury, protecting the function of important organs in the patient s undergoing mitral valve replacement
