摘要
目的了解慢性阻塞性肺病急性加重(acute exacerbation of chronic obstructive pulmonarydisease,AECOPD)患者生活质量的现状,分析应对方式对生活质量的影响。方法采用问卷调查法,对2016年12月—2017年12月于福州市某三级甲等综合性医院的135例AECOPD患者进行研究调查。问卷包括:一般资料信息表、生活质量量表(the MOS 36-item Short Form Health Survey,SF-36)和医学应对问卷(Medical Response Questionnaire,MCMQ)。结果 AECOPD患者生活质量低于六省常模(P<0.01),屈服或接受应对方式和生命力(Vitality,VT)、社会功能(Social Functioning,SF)、总体健康(General Health,GH)、精神健康(Mental Health,MH) 4个维度均呈负相关(P<0.05),面对或斗争的应对方式与生理职能(Role-Physical,RP)、精神健康(Mental Health,MH)呈正相关(P<0.05)。结论患者的生活质量较低,应对方式在一定程度上对患者的生活质量有影响,因此在临床护理工作中,引导患者尽量避免采用屈服或接受的应对方式,鼓励患者在治疗期间采用积极面对的方式,配合治疗和相应的护理,以提高患者的生活质量。
Objective Understand the quality of life of patients with acute exacerbation of chronic obstructive pulmonary disease( AECOPD),and analyze the impact of coping styles on quality of life. Methods A total of 135 cases of AECOPD patients in a tertiary Grade A comprehensive hospital in Fuzhou were investigated by using questionnaire survey. The questionnaire includes: General Information Sheet,the MOS 36-item Short Form Health Survey( SF-36) and Medical Response Questionnaire( MCMQ). Results The quality of life of AECOPD patients was lower than the norm of six provinces( P< 0.01),yield or acceptance of coping style was negatively correlated with Vitality,social functioning,general health,mental health in all four dimensions( P<0.05),and the coping style of confrontation or struggle was positively related to Role-Physical and Mental Health( P<0.05). Conclusions The quality of life of AECOPD patients is low,and coping styles have an impact on the quality of life of patients to a certain extent. Therefore,in clinical nursing work,patients should be guided to avoid the coping styles with yielding or receiving,while a positive attitude should be encouraged during the treatment,which may improve the quality of life.
