摘要
目的为提高疗效和减少手术创伤,采用小切口开窗减压人工髓核置换治疗腰椎间盘突出症并观察临床效果。方法41例患者采用小切口开窗减压髓核摘除后植入人工髓核治疗腰椎间盘突出症。结果41例患者术后下肢主诉症状完全缓解;6例患者仍有轻微的感觉异常;8例患者伸拇肌力以及4例患者腓骨长短肌肌力由Ⅳ级恢复至Ⅴ级,4例患者伸拇肌力由Ⅲ级恢复至Ⅳ级;6例患者残存轻微腰痛,1例患者出现中度腰痛,通过固定、服用消炎镇痛药后症状缓解。结论该手术操作简单,有切口小,手术时间短,不增加患者痛苦,疗效确切,后遗症少。
Aim To observe the clinical effect of small incision and fenestration and prosthetic disc nucleus replacement in the treatment of lumbar intervertebral disc herniation. Methods Forty one patients with lumbar intervertebral disc herniation, 25 males and 16 females, including 22 cases of L4～ 5 and 19 cases of L5～ S1, were treated with small incision and fenestration and prosthetic disc nucleus replacement. Results The chief symptom of lower limbs was of remission in 41 cases of lumbar intervertebral disc herniation after treatment. 6 out of 41 cases had light paresthesia, 8 out of 41 cases recovered from Ⅳ grade to Ⅴ grade in the muscle strength of extensor hallucis longus , 4 out of 41 cases recovered from Ⅳ grade to Ⅴ grade in the muscle strength of long and short peroneal muscle, 4 out of 41 cases recovered from III grade to IV grade in the muscle strength of extensor hallucis longus , 6 out of 41 cases had light lumbag, and 1 out of 41 cases had moderate lumbag. The symptoms above were of remission by fixation and antiphlogistic and analgesic.Conclusion Simple operative procedure, short time, no distress, certain efficacy and few sequelea are the advantages of this operation.
