摘要 目的 :探讨微小病变型肾病综合征 (MCNS)外周血单个核白细胞 (PBMC)体外培养上清液中白细胞介素(IL) 18的表达与MCNS发病之间的关系及地塞米松 (DEX)对体外培养PBMC表达IL 18的影响。方法 :MCNS2 3例 ,激素治疗前、后 (尿蛋白转阴 )测定PBMC体外用植物血凝素 (PHA)刺激下培养 2 4h后上清液中IL 18的表达量。并将治疗前PBMC分为 2组 :PBMC +PHA组 13例 ,PBMC +PHA +DEX组 13例 ,对照组为 2 2例年龄相仿的健康自愿者和 18例呼吸道感染病例。结果 :(1)各组IL 18测定值分别为 :治疗前PBMC +PHA组 (414 .5± 112 .1)ng/L ,PBMC +PHA +DEX组 (2 0 2 .1± 76 .3)ng/L ,治疗尿蛋白转阴后 (182 .3± 6 7.2 )ng/L ,呼吸道感染组PBMC+PHA(190 .1± 87.5 )ng/L ,正常对照组PBMC +PHA(16 4 .8± 86 .4 )ng/L。①MCNS治疗前后IL 18的表达量前者显著高于后者 (P <0 .0 0 1)。②MCNS治疗前加DEX组IL 18表达量显著低于不加DEX组 (P <0 .0 0 1)。③治疗尿蛋白转阴后IL 18的表达量与正常对照组相似 ,无显著性差异 (P >0 .0 5 )。④呼吸道感染组与正常对照组之间也无显著性差异 (P >0 .0 5 )。 (2 )MCNS患儿PBMC培养上清液中IL 18量与 2 4h尿蛋白定量呈正相关 (P <0 .0 5 ) ;与血浆白蛋白浓度呈负相关 (P <0 .0 5 )。结论 : Objectives:To investigate the relationship between the pathogenesis of minimal change nephrotic syndrome(MCNS)and expression of interleukin (IL) 18 by peripheral blood mononuclear cell(PBMC),and the effect of dexamethason(DEX)to expression of IL 8 in PBMC.Methods:PBMC of 23 patients with MCNS that was stimulated with phyto hemagglutinin(PHA) was cultured for 24 hours in vitro.IL 18 levels in supernatant was measured by enzyme linked immunosorbent assay(ELISA)before and after treatment with prednison. Before treatment,PBMCs were divided into two groups, based on treated with or without DEX.Normal control group consisted of 15 healthy volunteer of similar age. Infection group consisted of 18 patients with respiratory infection.Results:(1)IL 18 level in supernatant of prednison pretreatment was markedly higher then that of prednison treatment [(414.5±112.1)ng/L,vs(182.3±67.2)ng/L,( P < 0.001 )];(2)IL 18 level in supernatant of MCNS prednison pretreatment with DEX group was lower than that of without DEX[(202.1±76.3)ng/L vs (414.5±112.1)ng/L,( P <0.001)];(3)IL-18 level in supernatant prednison treatment was similar to that of normal control group[(182.3±67.2)ng/L vs(164.8±86.4)ng/L,( P >0.05)];(4) there was no significant difference between the respiratory infection group and normal control group( P >0.05);(5) there was a positive correlation between the IL 18 levels in supernatant of cultured PBMC prednison pretreatment in vitro and 24 hours urinary protein,(r=0.687, P < 0.05 );and (6)there was a negative correlation between the IL 18 levels in supernatant of cultured PBMC prednison pretreatment in vitro and albumin in serum(r=-0.710, P < 0.05 ).Conclusion:(1)The pathogenesis of MCNS is related to IL 18;(2) DEX may inhibit the expression of IL 18 of PBMC in vitro;(3) Infection didn't affect the expression of IL 18 of PBMC.
