摘要
目的:研究颅高压中脑脊液脉动压的变化及其规律。方法:14条狗安置硬膜外球囊并注液制成颅高压模型。通过改变球囊内液体量改变颅高压程度和颅内容积。通过压力传感器持续记录脑室内压力、腰椎管内压力及体动脉压。结果:随着颅内压的持续增高,脑脊液动脉压也相应增高,两者间呈正性线性关系。当排空球囊内液体后再次注入液体时,颅内压和脑脊液脉动压增大的幅度较首次球囊内注液时更加明显。颅内压高到一定程度后,颅压描记中持续出现≤0.2 Hz的波动(M波)。出现该波动时,颅内压和脑脊液脉动压增大的幅度较未出现该波时更加明显。结论:脑脊液脉动压随着颅内压的增高而增高,两者间呈正性线性关系。同一观测对象如果出现脑脊液脉动压变化趋势改变,提示颅内顺应性改变。M波的出现可能提示颅内压代偿机制受损。
Aim: To investigate the variation and regularity of cerebrospinal fluid pulse pressure during the process of increasing intracranial pressure. Methods: Fourteen dogs were installed epidurally with latex sacculus to establish models of increased intracranial pressure. The degree of intracranial pressure and volume can be altered by changing the volume of fluid in the sacculus. During the process, pressure transducers were arranged to monitor and record the systematic blood pressure and the variations of the pressure of intracranial .ventricle and lumbar subarachnoid cavity. Results: With the continual increasement of intracranial pressure, cerebrospinal fluid pulse pressure correspondingly increased, which showed a positive linear relationship with the change of intracranial pressure. After empting and reinfusing the sacculus, the increasement amplitude of intracranial pressure and cerebrospinal fluid pulse pressure appeared more evident than that during the originally infusing process. Above some certain degree of increasement of intracranial pressure, a mechanically unknown pusle wave with its frequency no more than 0.2 Hz showed continually. With the unknown wave, the increasement amplitude of intracranial pressure and cerebrospinal fluid pulse pressure enlarged more evidently than that without the wave. Conclusion: The increasement of cerebrospinal fluid pulse pressure shows a positive linear relationship with that of intracranial pressure. To the same observed object, if the variation tendency of cerebrospinal fluid pulse pressure changes, it may show some changes of the intracranial complicance. The appearance of the unknown wave may reveal the injury of the compensation mechanism of intracranial pressure.
