摘要
目的 通过对潜艇艇员口腔卫生和口腔幽门螺杆菌感染的调查 ,探讨复发性口腔溃疡(recurrent oral ulcer,ROU)在潜艇人员中发生率高的原因和幽门螺杆菌 (Helicobacter pylori,H.pylori)在潜艇艇员复发性口腔溃疡发病中的意义。方法 对 36 4名潜艇艇员进行口腔卫生和 ROU患病情况调查 ,并通过 1min尿素酶试剂盒检查所有艇员口腔幽门螺杆菌感染情况。随机抽取 6 0例 ROU患者 (A组 )和 6 0例非 ROU艇员 (B组 ) ,通过聚合酶链反应 (polymerase chain reaction,PCR)对其口腔粘膜病损区 (A组 )或正常口腔粘膜 (B组 )、牙菌斑 H.pylori进行检测 ,并通过胃镜检查两组中胃病患病情况。结果 36 4名潜艇艇员中 ,口腔卫生优良率仅为 2 2 .0 % (80 /36 4 ) ,ROU 的发生率高达 6 6 .8% (2 4 3/36 4 ) ,艇员口腔幽门螺杆菌 1m in尿素酶试剂盒检测阳性率 76 .6 % (2 79/36 4 )。A组中 ,5 2例口腔粘膜病损区、4 4例正常口腔粘膜、4 9例牙菌斑 H.pylori检测阳性 ,阳性率分别为 86 .7% ,73.3%和 81.7% ,与B组相比差异具有高度显著性 (P<0 .0 1) ;A组中 5 8例胃镜检查有不同程度的慢性胃炎 ,B组仅 2 2例 ,两组相比差异有非常显著性 (P<0 .0 1)。结论 口腔 H.pylori与 ROU的发病有关 ;潜艇环境口腔卫生较差导致口腔 H.
Objective To investigatie the cause of high ocourence and reccurence of recurrent oral ulcer (ROU) and the relationship between ROU and Helicobacter pylori (H.pylori) in the submariners.Methods 364 male submariners were surveyed their oral hygiene and oral health habit,and with 1 min H.pylori box examined their oral H.pylori.Then 60 ROU submariner patients and 60 submariner controls were randomly selected and studied.Polymerase chain reaction (PCR) was applied to detect H.pylori in the oral mucosa ucler the lesion (group A) or the normal lesion( group B) and also in the dental plaques.Gastroscopy was used to examine the gastric diseases of the two groups.Results Of the 364 submariners the rate of best and good oral hygiene was 22.0%,and 243 of those submariners were ROU patients(66.8%).The rate(with 1 min H.pylori box) of positive oral H.pylori was 76.7%.52 of 60 ROU cases had positive H.pylori in oral mucosa ucler lesion,with 44 in normal oral mucosa and 49 in dental plaques,and there was significant difference comparing with the control group (p<0.01).Besides,58 of ROU group and 22 of control group were having different types of chronic gastritis,and they were also significantly different(P<0.01).Conclusions Submarine condition could favor the oral growth of H.pylori in the submariners and bad oral hygien might help the H.pylori colonization.H.pylori is perhaps one of the pathogenic factor causing ROU.
