新生儿窒息并脑损害血小板参数的变化及其临床意义 被引量:1

The clinical significance of platelet parameter changes in neonatal asphyxia with brain damage

摘要 目的 :探讨新生儿窒息并脑损害的血小板参数的变化及临床意义。方法 :对 1999年 5月~ 2 0 0 3年 1月被确诊为新生儿窒息并脑损害 5 0例患儿的血小板数 (PLT)、平均血小板体积 (MPV )及血小板分布宽度 (PDW )进行动态观察 ,并与 3 0例窒息无合并脑损害组及 3 0例正常新生儿对照组比较 ;脑损害组中合并颅内出血组 3 0例血小板参数与无颅内出血组 2 0例作比较。结果 :①新生儿窒息并脑损害组急性期 (出生后第 1天 )较无合并脑损害组及正常新生儿对照组明显降低 (P <0 0 1) ,而MPV和PDW与无脑损害组及正常对照组比较明显增高 ,差异显著 (P <0 0 1)。在恢复期 (生后第 7天 ) ,三组的PLT、MPV和PDW之间的比较均无明显差异 (P >0 0 5 )。②急性期脑损害组中合并颅内出血组PLT较无颅内出血组明显降低 (P <0 0 1) ,而MPV和PDW较无颅内出血组明显增高 (P <0 0 5 )。结论 Objective To explore the clinical effectiveness of the platelet parameter in neonatal asphyxia with brain damage.Methods From May,1999 to Jan,2003,50 cases of neonatal asphyxia with brain damage were final diagnosed, the platelet total(PLT), mean platelet volume(MPV) and platelet distribution width(PDW) were dynamic observed. It was compared with 30 cases of brain non-damage and 30 cases of normal term infant. In the cases of brain damage, the platelet parameter in the group with intracranial hemorrhag(ICH) was compared with the group without ICH. Results ①PLT in acute period (one day after birth) of neonatal asphyxia with brain damage was obviously lower than that of the non-damage and the normal term infant (P<0 01),MPV and PDW were obviously high than that of the non-damage and the normal term infant(P<0 01). PLT, MPV and PDW in the recovery period(seven days after birth) were no significantly different between the three groups(P>0 05).②In the cases of asplyxia with brain damage, PLT in the cases with ICH was obviously lower than that of the non-ICH(P<0 01),MPV and PDW were obviously high than that of the non-ICH(P<0 05) during acute period.Conclusion It's a marker of evaluating the prognosis of neonatal asphyxia with brain damage and eraly intracranial hemorrhage proneness by dynamic observing the platelet parameter.
