主动脉瓣狭窄患者能量损失及与主动脉内径的关系研究

摘要 目的探讨先天性二瓣叶主动脉瓣狭窄(AS)患者与升主动脉内径及左心室质量的关系,血流经过狭窄瓣口后能量的损失(EL)及恢复(ER)。方法 136例患者,按照主动脉瓣口面积(AVA)和升主动脉内径分组,分别测量升主动脉内径、左心室质量、EL及ER,观察其与狭窄瓣口的关系。结果 AVA与升主动脉内径及左心室质量呈高度正相关关系,升主动脉径基本正常的患者ER显著,升主动脉扩张时,EL大,ER较小;导管AVA大于多普勒AVA,二者高度相关关系,导管AVA与能量损失系数(ELCo)相近,EL与狭窄两边导管测量的压力阶差(△P)呈正比,瓣膜狭窄越重,EL越大,ER与主动脉内径呈反比,主动脉内径基本正常患者,ER更明显。结论 AS可致升主动脉扩张及左心室质量增加,ELCo与导管AVA、EL与△P正相关;ER与升主动脉内径负相关。 Objective The study was about the relationship between the ascending aorta diameter and left ventricular mass in patients with bicuspid aortic stenosis,the energy loss(EL) and energy recovery(ER) when the blood passes through the narrow aortic valve. Methods 136 patients were divided into groups by aortic valve orifice area(AVA) and ascending aorta, to measure the ascending aorta diameter and left ventricular mass, EL,ER. To observe the relationships between them and aortic valve stenosis. Results Aortic valve orifice area is highly correlated to the inner diameter and left ventricular mass, ascending aorta diameter in patients with normal pressure recovery are significant. The AVA catheter is larger than the AVA doppler, which are highly correlated, the AVA catheter is the same as ELCo. EL is proportional to the pressure difference(△ P), the valve heavier, the EL greater. ER is negatively correlated with the ascending aorta. In patients with normal ascending aorta, ER was more pronounced. Conclusion Aortic stenosis may liad to the expansion of the ascending aorta and the left ventricular mass increase. ELCo is associated with the AVA catheter, EL with△P ER is negative correlation with the ascending aorta.
