摘要
目的探究脊柱定点旋扳复位手法联合小针刀整体松解术对腰椎间盘突出症(LDH)患者疼痛及腰椎功能(JOA)评分的影响。方法选取2015年6月-2017年4月周口市东新区人民医院LDH患者86例,依据随机数表法分组,各43例。对照组行小针刀整体松解术治疗,实验组在对照组基础上行脊柱定点旋扳复位手法治疗。两组均连续治疗3周,对比两组治疗效果及治疗前后疼痛程度(VAS)及JOA评分,随访1年,统计两组复发率。结果实验组治疗总有效率97.67%(42/43)较对照组81.40%(35/43)高(P <0.05);两组VAS、JOA评分治疗前相比,无显著差异(P>0.05);实验组治疗后VAS评分较对照组低,JOA评分较对照组高(P <0.05);实验组随访1年复发率较对照组低(P <0.05)。结论采用小针刀整体松解术治疗LDH同时,联合采用脊柱定点旋扳复位手法,可有效缓解患者疼痛,显著改善腰椎功能,进一步提高治疗效果,且复发率较低,在临床治疗中具有一定的意义。
Objective To explore the effect of fixed-point rotation and reduction technique combined with acupotomy on pain and lumbar function(JOA)score of patients with lumbar disc herniation(LDH).Methods 86 LDH patients from Dongxin District People’s Hospital of Zhoukou City from June 2015 to April 2017 were selected and divided into two groups according to random number table method,43 patients in each group.The control group was treated with overall acupotomy,while the experimental group was treated with fixed-point rotation and reduction technique of spine on the basis of the control group.Both groups were treated continuously for 3 weeks.The therapeutic effect,pain degree(VAS)and JOA score before and after treatment were compared between the two groups.After follow-up for 1 year,the recurrence rate of the two groups was compared.Results The total effective rate of the experimental group was 97.67%(42/43),higher than that of the control group81.40%(35/43)(P<0.05).There was no significant difference in VAS and JOA scores between the two groups before treatment(P>0.05).After treatment,the VAS score of the experimental group was lower than that of the control group,and the JOA score was higher than that of the control group(P<0.05).The recurrence rate of the experimental group was lower than that of the control group(P<0.05).Conclusion The treatment of LDH with overall acupotomy combined with spinal fixed-point rotation reduction technique can effectively relieve the pain of patients,significantly improve lumbar function,further improve the treatment effect,and the recurrence rate is low,which has certain significance in clinical treatment.
