摘要
目的:探讨原发性肾脏弥漫大B细胞淋巴瘤(DLBCL)的临床特征及诊治方法。方法:回顾性分析我院2011年8月～2016年9月收治的11例原发性肾脏DLBCL患者的临床资料,患者年龄23～72岁,平均57.2岁,首发症状均表现为患侧腰痛,3例患者伴有发热。11例患者均行手术治疗。结果:术后病理均为原发性肾脏弥漫大B细胞淋巴瘤,其中生发中心细胞来源3例,非生发中心细胞来源8例。Ann Arbor分期:ⅠE期4例,ⅡE期4例,ⅢE期1例,ⅣE期2例。免疫组化检查CD20、CD79a均呈阳性表达,CK呈阴性表达。其中9例患者术后辅助化疗。随访5～49个月,中位随访时间21个月,其中6例存活,5例死亡。结论:原发性肾脏DLBCL较为罕见且预后较差,组织病理学可明确诊断。手术联合化疗可提高患者预后。
Objective:To investigate the clinical features,diagnosis and treatment of primary renal diffuse large B-cell lymphoma.Method:The retrospective study was conducted at First Affiliated Hospital of Zhengzhou University and comprised records of 11 patients who were diagnosed as primary renal diffuse large B-cell lymphoma from August 2011 to September 2016.The median age was 57.2(range,23-72)years old.Flank pain was the initial symptom and 3 cases had fever.All patients underwent surgery successfully.Result:All patients were diagnosed as primary renal diffuse large B-cell lymphoma after operating.Three cases was GCB-DLBCL and the remaining 8 cases were non-GCB-DLBCL,according to Hans algorithm.And they were classified as Ann Arbor stageⅠE(n=4),stageⅡE(n=4),stageⅢE(n=1),stageⅣE(n=2).Immunohistochemical staining analysis showed that CD20,CD79 awere positive and CK was negative.Nine cases received adjuvant chemotherapy.The median follow-up period was 21(range,5-49)months.Six cases survived at the end of the study.Five cases died during the follow-up period.Conclusion:Primary renal diffuse large B-cell lymphoma is a rare tumor with poor prognosis.The gold standard for diagnosing it is histological features and immunohistochemical staining.Surgery combined with chemotherapy can improve the prognosis.
