2型糖尿病伴嗅觉功能障碍患者的静息态fMRI初步研究

Resting-State fMRI Preliminary Studies of Type 2 Diabetes Mellitus Patients with Olfactory Dysfunction

摘要 目的利用静息态功能磁共振成像(rs-fMRI)探讨2型糖尿病(T2DM)伴嗅觉功能障碍患者脑功能异常改变及其与嗅觉功能评分的相关性。方法搜集符合糖尿病诊断标准和入组标准的T2DM患者32例,并根据临床嗅觉功能评分将其分为伴嗅觉功能障碍组(ODM)13例和无嗅觉功能障碍组(DM)19例。所有患者均行3.0 T MRI设备rs-fMRI扫描,利用REST软件计算被试者局部一致性(ReHo)与低频振幅(ALFF)值,并与嗅觉功能评分行相关性分析,其他资料采用SPSS 21.0软件进行统计学分析,以P<0.05为差异具有统计学意义。结果 ODM与DM组之间人口学资料差异无统计学意义(P>0.05),嗅觉功能评分差异有统计学意义(P<0.05)。两组比较ALFF值减低脑区为双侧额上、中回眶部、直回、嗅皮质及右侧杏仁核;增加脑区为双侧额上回及右侧额中回。两组比较ODM组ReHo值普遍减低;增高脑区为双侧辅助运动区、额上回及右侧额中、下回。ODM组ALFF值与嗅觉评分呈正相关脑区双侧额上回内侧、直回,左侧额上回眶部、颞中回、枕中回和距状回,右侧颞中回和楔前叶(P均<0.05);呈负相关脑区为双侧小脑后叶(P<0.05)。结论 T2DM伴嗅觉功能障碍者存在以嗅觉功能脑区为主的多个脑区异常,rs-fMRI技术ALFF与ReHo值可定量评估该类患者脑损伤的部位及程度,具有潜在的临床应用价值。 Objective To explore the changes of brain function in the type 2 diabetes mellitus(T2 DM) with olfactory dysfunction and the relationship between the brain function and olfactory test score by using Resting-State functional magnetic resonance imaging(rs-fMRI). Methods 32 cases of T2 DM patients who met the diagnostic criteria of diabetes mellitus and inclusion criteria were collected. According to the clinical olfactory function score,T2 DM patients were divided into olfactory diabetes mellitus(ODM) group(13 cases) and non-olfactory dysfunction group(DM) group(19 cases).All subjects underwent rs-fMRI scan by a 3.0 T MRI scanner. Regional homogeneity(ReHo) and amplitude of low frequency fluctuation(ALFF) were calculated with the software of REST. Then we analyzed the relationship between the ALFF and olfactory test score in ODM group.Other data were analyzed by SPSS 21.0.P<0.05 indicated statistically significant difference. Results Between ODM and DM group,there were no significant difference in demographic data(P>0.05),yet there were significant difference in olfactory function score(P<0.05).Compared to DM,ALFF in ODM group decreased in orbital of bilateral superior frontal gyms and middle frontal gyrus,bilateral rectus gyrus,bilateral olfactory cortex and right amygdala,increased in bilateral superior frontal gyrus and right middle frontal gyrus. ReHo were generally reduced in ODM group. ReHo in ODM group increased in the supplementary motor area,bilateral superior frontal gyrus,right middle and inferior frontal gyrus.The positive correlation between the olfactory test score and ALFF was found in right precuneus,left middle occipital gyrus,left calcarine gyrus,bilateral middle temporal gyrus,orbital of left superior frontal,bilateral rectus gyrus and superior frontal gyrus(P> 0.05);the negative correlation was found in bilateral posterior cerebellar lobe(P<0.05). Conclusion There were multiple brain abnormalities in T2 DM with olfactory dysfunction,especially in the area of olfactory function,ALFF and ReHo valu
