摘要
目的 探讨抑郁症 (MD)和广泛性焦虑症 (GAD)病人在免疫学方面的差异性。方法 采用酶联免疫吸附法 (ELISA)检测 38例MD病人 (抑郁组 )、30例GAD病人 (焦虑组 )、30名健康人 (正常对照组 )的血清白细胞介素 2 (IL 2 )、白细胞介素 6 (IL 6 )、白细胞介素 1β(IL 1β)、白细胞介素 8(IL 8)、可溶性白细胞介素 6受体 (SIL 6R)、肿瘤坏死因子 α(TNF α)水平 ,使用汉密顿抑郁量表 (HAMD)和汉密顿焦虑量表 (HAMA)对抑郁组和焦虑组病人进行评定。结果 抑郁组IL 2、SIL 6R、IL 8水平高于正常对照组 (P <0 0 5 ) ;焦虑组SIL 6R、IL 8水平高于正常对照组 (P <0 0 5 ) ,IL 6水平低于正常对照组 (P <0 0 5 )。焦虑组和抑郁组比较 ,焦虑组SIL 6R水平高于抑郁组 (P <0 0 5 ) ,IL 2 ,IL 6低于抑郁组 (P <0 0 5 )。抑郁组和焦虑组的各种免疫学指标与精神病理学评分均无明显相关性。结论 抑郁症和广泛性焦虑性病人可能具有不同的免疫机制 ,血清IL 2、IL 6、SIL 6R的检测可能有助于二者的鉴别。
Objective To explore the difference of immunology between patients with major depression (MD) and with generalized anxiety disorder (GAD). Methods Serum levels of IL 2?IL 6?IL 1β?IL 8?SIL 6R?TNF α in depressive group, the anxious group and the normal controls were determined using methods of enzyme linked immuno sorbent assay (ELISA). Patients with MD and GAD were investigated by Hamilton Depression Scale (HAMD) and Hamilton Anxiety Scale (HAMA). Results (1)Serum levels of IL 2, SIL 6R, IL 8 were significantly higher in the depressive group than in the normal controls. (2) Serum levels of SIL 6R, IL 8 were significantly higher in the anxious group than in the normal control; while serum level of IL 6 in the former group was lower than the latter group. (3) Serum level of SIL 6R was higher in the anxious group than in the depressive group. Whereas serum levels of IL 2 and IL 6 in the former group was lower than the latter group. (4) Serum levels of various immunological cytokines were not related to psychopathological scores in both the depressive and anxious group. Conclusions Immunological mechanism of patients with MD may be different from patients with GAD. Measurements of serum levels of IL 2, IL 6, SIL 6R may be helpful to differentiate MD from GAD.
