神经外科围手术期脑血流动力学变化

摘要 目的 研究神经外科围手术期脑血流动力学变化,以期为麻醉和手术提供依据。方法 将29例神经外科手术患者,分成颅内手术组(A组)与脊柱、脊髓手术组(B组),应用经颅多普勒诊断仪(TCD)对术前、麻醉、术始、术中、术毕和术后六个时程连续监测脑血流速度。结果 麻醉后脑血流速度显著下降,峰值血流速度及平均血流速度分别下降22.78%和22.85%,搏动指数无显著性变化;TCD可监测手术对颅内压的影响、术后可能出现的并发症如颅高压和脑血管痉挛。结论 TCD可监测麻醉及手术对脑血流的影响,并可监测颅内压。 Objective To study the cerebral hemodynamic changes during peri - and - intra -operation for carrying out effective anesthesia and operation. Methods Twenty - nine neurosurgical patients under general anesthesia were divided into intracranial diseases group ( A) and spinal cord or spinal diseases group (B). The peak blood flow velocity (Vp) , mean blood flow velocity (Vm) and pulsatility index (PI) were observed continuously using TCD and recorded at six time intervals; preoperation, postinduc-tion of anesthesia, beginning of surgery, intraoperation, end of surgery and postoperation. Results There was a significant decrease in Vp and Vm by 22. 78% and 22.85% respectively, during general anesthesia. There was no significant change of PI. TCD can be used to monitor the operative effect on hemodynamic changes. The complications such as elevation of ICP and vasospasm after surgery can be detected early using TCD. Conclusion TCD can be used to evaluate the effect of general anesthesia and operation on cerebral hernodynamics, and to monitor ICP.
